Good evening. An inquest has heard how a grandmother died after a | :01:22. | :01:25. | |
surgeon accidentally tried to remove her liver instead of one of | :01:25. | :01:28. | |
her kidneys. The operation was supposed to help Amy Francis beat | :01:28. | :01:31. | |
cancer. Instead the routine operation using keyhole surgery | :01:31. | :01:41. | |
:01:41. | :01:44. | ||
ended in tragedy. Caroline Evans reports. Amy Francis was 77, a | :01:44. | :01:47. | |
mother of four with seven grandchildren. She had lived and | :01:47. | :01:52. | |
new -- worked in Newport, enjoying good health for most of her life, | :01:52. | :01:57. | |
until she was diagnosed with a kidney cancer. Doctors told her | :01:57. | :02:01. | |
that if she had her right kidney removed, there was a good chance | :02:01. | :02:06. | |
her left kidney could be treated and she would recover. It was a | :02:06. | :02:11. | |
great shock. It was what we thought would be a routine operation. We | :02:11. | :02:18. | |
were devastated at the time to lose her so suddenly. Surgeon Adon -- | :02:18. | :02:21. | |
Adam Carter had done such an operation more than 100 times | :02:21. | :02:28. | |
before. But what should have been routine went tragically wrong. Amy | :02:28. | :02:31. | |
Francis was undergoing keyhole surgery where small cuts are made | :02:31. | :02:35. | |
in the body, allowing surgeons to insert a camera and instruments | :02:35. | :02:39. | |
without the need for -- need for a bigger incision. The procedure | :02:39. | :02:44. | |
should have involved attaching the kidney and pulling it out through a | :02:44. | :02:51. | |
cat in the crime. By that time, the key hole camera cannot be used and | :02:51. | :02:56. | |
the sea -- surgeon is working by touch alone. Deliverer is usually | :02:56. | :03:05. | |
out of the way higher up -- the liver. When it be surgeon reached | :03:05. | :03:09. | |
in to get hold of the kidney, his hand instead grabbed the liver. He | :03:09. | :03:13. | |
told the inquest today that initially he had asked a trainee | :03:13. | :03:16. | |
surgeon to remove the kidney but when she said she was not confident, | :03:16. | :03:21. | |
he had taken over and he was responsible for the mistake. I | :03:21. | :03:25. | |
should have known where the kidney was will stop emerging from the | :03:25. | :03:31. | |
hearing, Amy Francis's family said Mr Carter's honesty had saved them | :03:31. | :03:37. | |
grief and the hospital from legal action. It was what we expected the | :03:37. | :03:42. | |
outcome to be, it was a mistake. But we are glad we came and | :03:42. | :03:47. | |
listened to the full story of what happened. We take heart from the | :03:47. | :03:51. | |
fact they have learned from the mistake that was made. We accept | :03:51. | :03:57. | |
the decision of the court. We miss -- which Mr Carter all the best and | :03:57. | :04:05. | |
appreciate his honesty in admitting the error. Hopefully we can put it | :04:05. | :04:11. | |
behind us now. In a statement, the health board said it fully accepts | :04:11. | :04:15. | |
the verdict and all thoughts are with the family. A full internal | :04:15. | :04:19. | |
investigation was undertaken which examined in every detail every | :04:19. | :04:23. | |
action taken prior to and during the operation. Every possible | :04:23. | :04:27. | |
effort was made by the surgical team to save the life of Amy | :04:27. | :04:33. | |
Francis, they say. The details of the death have been shared with | :04:33. | :04:36. | |
surgeons nationwide core stock as a result, it is hoped lessons are | :04:36. | :04:41. | |
being learned by the profession across Britain. Plans to put | :04:41. | :04:44. | |
primary schools in Wales into bands will go ahead later this year, | :04:44. | :04:48. | |
despite calls for the whole scheme to be abandoned. The system is | :04:48. | :04:50. | |
similar to the controversial one already in place in our | :04:50. | :04:53. | |
comprehensives, but as many as a third of primary schools in rural | :04:53. | :04:56. | |
areas may not be included. The Education Minister denies claims | :04:56. | :04:59. | |
that'll make the whole system meaningless, as our Education | :04:59. | :05:00. | |

In just a few months' time, parents, teachers and pupils here at | :05:03. | :05:06. | |
Greenway Primary School in Cardiff will learn for the first time what | :05:06. | :05:11. | |
band their school is in. Just like the system in secondary schools | :05:11. | :05:14. | |
across Wales, the banding is designed to see how each school is | :05:14. | :05:17. | |
performing and which ones need extra support. It's likely that | :05:17. | :05:20. | |
pupil attendance and teacher assessments at Key Stage 1 and 2 | :05:20. | :05:24. | |
will be taken into account. The exact criteria is still being | :05:24. | :05:32. | |
discussed. But it's controversial. I don't think we need it. I think | :05:32. | :05:36. | |
it might be divisive, it's a score on the door and doesn't show what | :05:36. | :05:40. | |
goes on behind that door. It's one number. I think we have other | :05:40. | :05:43. | |
systems in place, Estyn framework and the inspection system that pick | :05:43. | :05:49. | |
up schools doing well and schools that need help. But unlike Greenway | :05:49. | :05:51. | |
Primary School, Ysgol Penllwyn near Aberystwyth probably won't be | :05:51. | :05:57. | |
included in the new system. In fact, around a third of schools in Wales | :05:57. | :06:00. | |
won't because they have too few pupils to be accurately and fairly | :06:00. | :06:07. | |
measured. With a relatively small number of kids in each year group, | :06:07. | :06:10. | |
one child can represent aboutt 33% of that particular cohort so that | :06:10. | :06:13. | |
would unbalance the figures year on year and make it very difficult to | :06:13. | :06:23. | |

make them meaningful. Parents here told us they didn't need to be part | :06:24. | :06:30. | |
of a banding system to know their school was doing well. We know that | :06:30. | :06:33. | |
our child is doing very well here, academically, and other parents are | :06:33. | :06:38. | |
happy too, so we're fine with that. This is a small school. Community | :06:38. | :06:41. | |
and family spirit far outweighs anything else at this stage of the | :06:41. | :06:50. | |
game at primary school level. of the parents seem very happy, we | :06:50. | :07:00. | |

not only touched people in the Swansea Valley and Wales, but was | :11:14. | :11:17. | |
felt around the world. In the months since September, the | :11:17. | :11:19. | |
trustees have received cheques from right across the globe. There's | :11:19. | :11:21. | |
been particular generosity from countries with strong mining | :11:21. | :11:24. | |
traditions, such as New Zealand and Chile. Today the trustees met to | :11:24. | :11:28. | |
monitor how the fund is doing. They've already invested a | :11:28. | :11:31. | |
substantial part in an account to gain interest and two payouts have | :11:31. | :11:34. | |
already been made. One was to the families to help with funeral costs, | :11:34. | :11:38. | |
the other to the daughter of one of the men who died. She is disabled, | :11:38. | :11:41. | |
and has used the money to buy an electric wheelchair and a specially | :11:41. | :11:47. | |
adapted car. We have had numerous fundraisers over the Christmas.. | :11:47. | :11:52. | |
is phenomenal. The fund is expected to be wound up later this year. | :11:52. | :11:55. | |

current treatment really only works for a small number of patients, and | :14:02. | :14:08. | |
the drug that has just been approved -- the drugs that have | :14:08. | :14:13. | |
been approved to not cover all types of the disease. This drug | :14:13. | :14:17. | |
will cover all the types of hepatitis C so there is a chance | :14:17. | :14:22. | |
