Good evening. An inquest into the death of a

:01:25.:01:27.

seven-year-old boy after heart surgery in Bristol has heard his

:01:28.:01:30.

father's claims he repeatedly pleaded with staff to take his son's

:01:31.:01:34.

condition more seriously. Luke Jenkins from Cardiff died after

:01:35.:01:36.

being discharged early from intensive care onto a cardiac ward

:01:37.:01:40.

at Bristol Children's Hospital. A hospital review at the time said

:01:41.:01:43.

there were not enough nurses on ward 32, and a failure to recognise his

:01:44.:01:51.

deteriorating condition. Luke Jenkins from Cardiff was born

:01:52.:01:55.

with only half of his heart working properly but despite his difficult

:01:56.:02:00.

start to life, he wanted to be a doctor, to help people like him. His

:02:01.:02:04.

cardiologist said he was deeply affected by the seven-year-old

:02:05.:02:14.

willpower and determination. The surgery went well but the following

:02:15.:02:17.

day, his parents notice something was wrong and asked doctors to move

:02:18.:02:24.

him back to intensive care. He started deteriorating and we were

:02:25.:02:30.

raising concerns. He could not even get out of bed. He was completely

:02:31.:02:36.

drained. With as raising concerns, they were not listening. Luke later

:02:37.:02:42.

died at the developing brain injuries, losing nearly two litres

:02:43.:02:47.

of blood. His parents had to make the agonising decision to turn off

:02:48.:02:50.

his life-support. A report by the health board later found there was a

:02:51.:02:52.

failure to listen to concerns. The health board says these problems

:02:53.:03:04.

have now been addressed and have since published a second report. It

:03:05.:03:11.

has been extreme difficulty. They had been waiting for this inquest to

:03:12.:03:14.

get to the bottom of what has happened to their son. His parents

:03:15.:03:21.

are still waiting for answers as the inquest into his death started

:03:22.:03:24.

today. His father was too unwell to be there. Luke 's father described

readily having to ask for painkillers for his son. When he

questioned how much blood was being drained from Luke 's chest after his

operation, he was told not to worry. The inquest into his death is due to

Wales' most vulnerable children are at risk of being targeted by

when she was 13 and moved more than ten times in the five years she was

in the system. While she was in care, she took drugs, slept in parks

and on one occasion, drank so much she was hospitalised. I felt like I

was being rescued but then I realised that it is exactly the same

as being at home. I thought that me being taken away from my mother

would give me a better life but it actually made it worse. On one

occasion, Lucy thought she would be moving to another home but actually

she was taken to a car park and told she would be moving to Manchester.

I refused at first but then they told me, if I did not go with these

people, the police would take me in handcuffs to Manchester. I had no

:08:47.:08:51.

choice. They took me to a residential home and they spoke to

:08:52.:08:57.

the workers there. And after about ten minutes, they just left me

:08:58.:09:02.

there. I just had the clothes on my back and the contents of my handbag.

They had ?11 for me to buy some clothes. She was 14 at the time. She

says she was not visited once by a social worker during the time she

and she's not alone. There are around 6000 children in

looked after properly. However, councils often flout guidance and

there are fears that children are becoming lost in the system. There

is a lot of evidence to suggest they are very vulnerable and at risk. A

lot of the time, they are placed in private residential homes, again we

:09:53.:09:58.

know that often these establishments are targeted by paedophiles and

:09:59.:10:03.

again, people just want to be accepted. I think they can be very

:10:04.:10:10.

vulnerable. The Association of directors of social services Cymru

:10:11.:10:13.

denies that these children are neglected or lost in the system

:10:14.:10:17.

editors say that there are inconsistencies when it comes to

:10:18.:10:20.

note local authorities -- but it does say. The Welsh local government

:10:21.:10:23.

Association says that reducing unnecessary out of area at

:10:24.:10:28.

placements continues to be a priority.

:10:29.:10:32.

A disproportionate number of children were targeted by paedophile

:10:33.:10:38.

gangs recently. One of the five victims of the grooming wreck --

:10:39.:10:43.

networking Rochdale had been placed in a home by the local authority.

:10:44.:10:50.

But for Lucy, life has improved. She wants to make sure what happened to

:10:51.:10:53.

her does not happen to another child. Children are lost in the

:10:54.:10:57.

system and they are neglected. I am one of the better off ones. Some of

my friends from care are in really bad positions, as we speak. They

:11:04.:11:07.

don't get any help. Helen Mary Jones is the Chief Executive of Youth

Cymru, and a former chair of the Children's Committee in the

Assembly. This is quite worrying isn't it, so what should be done

immediately now to reduce the risk for these children in care?

Immediately, I think all county councils in Wales need to review

:11:20.:11:22.

practices. It simply is not good enough to say there are

:11:23.:11:25.

inconsistencies. There is Cleo national guidance. There is a need

:11:26.:11:35.

for a joint plan. It is not a difficult thing to do. Then we need

:11:36.:11:38.

to look further down the line at some of the capacity issues and some

:11:39.:11:43.

of the things we need to do to make sure that children do not have to be

:11:44.:11:47.

moved. It is a J compact problem. There are not enough carers, there

:11:48.:11:54.

are not enough social workers and local authorities do not follow the

:11:55.:12:00.

rules. There are complexities, we know that we need to recruit more

foster carers, Welsh speaking foster carers, we know that we need more of

:12:05.:12:11.

them. The Welsh Government is taking wolves to create a single national

fostering and adoption service for Wales. That will help because we

In the end, this is a question of priorities. Local authorities have

known for years what they need to do about this. These children are

really vulnerable or they would not be in the system in the first place.

It is not good enough for them to say things are inconsistent. There

information. And they need to be much more proactive. Inconsistencies

downgraded from a 24-hour station to one that is only open during the

cuts be passed on to the service in the next two years, we will be

And what would the impact will that be # basically, if you close a fire

station or downgrade a fire station, you will be increasing the

response time, which will mean it will take longer for I fire station

to get -- we are in a time of cuts. The fire service has to make cuts.

Tough decisions need to be taken. Yes, they do need to be taken and I

understand we are in times of austerities. My answer to that would

HSBC is closing three of its branches in North Wales next

Tiley was just 24 years old when the South Wales miners walked out on

strike in 1984. His photographs were published around the world and

became some of the defining images of the decade. Now, he's returned to

:22:42.:22:45.

the people and places he documented nearly 30 years ago to explore the

:22:46.:22:50.

legacy of that strike. Here's Carwyn Jones.

March 1984, the minds and communities across South Wales were

:22:57.:23:02.

about to be transformed for ever. The year-long strike that followed

:23:03.:23:06.

was bitter and brutal. A young photographer from the Gwent valleys

captured every angle of the dispute. Not from afar, but up close and

:23:10.:23:14.

personal. In 1984, I was 24 years old. It was a massive new story

:23:15.:23:22.

breaking basically in my backyard. And I was starting to work for

:23:23.:23:24.

national newspapers, producing photographs. It was a real learning

:23:25.:23:30.

curve for me at the time to be thrown into this cauldron, if you

:23:31.:23:37.

like. Today, Roger has returned for a new photographic project. He is

revisiting the people and places he documented 30 years ago. His first

stop is the site of the old colliery. This is where the canteen

was. Just up the way there, a few hundred yards, was the shafts and

the pithead winding gear. It is just derelict now. Roger was there on the

cold morning of March in 1885 when the miners marched back to work. He

knew these men well. He had photographed them before and during

:24:11.:24:13.

the strike. Now, they are in his viewfinder again. His new series of

:24:14.:24:22.

portraits reveal that the men retain vivid memories of the strike. You do

:24:23.:24:37.

not have the community spirit now, I don't think, like you had before.

:24:38.:24:41.

Everybody knew each other. We worked with each other, we were belonging

:24:42.:24:45.

to each other. That has all gone now. It is not the same, I don't

:24:46.:24:51.

think so. For this retrospective project,

Roger is using the same black and white film stock he captained his

camera bag in 1984. His photographs will be exhibited next year to mark
