Carmarthenshire. Susan Hackett was walking with her daughter and eight | :01:20. | :01:22. | |
dogs on Cefn Sidan beach yesterday when they got into difficulty. | :01:23. | :01:26. | |
Lifeboat teams also dealt with two other incidents over the weekend. | :01:27. | :01:32. | |

but just behind me you can see a stretch of sand were the seagulls | :01:44. | :01:49. | |
are now. Within a few minutes that was quite a large sandbank but has | :01:50. | :01:52. | |
now completely disappeared because the tide is coming in at such a fast | :01:53. | :01:58. | |
rate and that does illustrate the danger of how you can get cut off | :01:59. | :02:02. | |
field without realising and it was just beyond the peer over there that | :02:03. | :02:07. | |
40-year-old Emma Hackett and 68-year-old Susan Hackett were | :02:08. | :02:10. | |
walking their eight dogs when they became stranded. The lifeboat | :02:11. | :02:16. | |
station is over there and they were here within five minutes but it was | :02:17. | :02:20. | |
then too late to save everyone. The two women had headed out from | :02:21. | :02:29. | |
Cambrai towards Burry Port. By late afternoon, the Mother's Day treat | :02:30. | :02:33. | |
turned to panic when they were stranded on a sandbank as the tide | :02:34. | :02:38. | |
rushed in. The RNLI was called by someone on the beach who saw them in | :02:39. | :02:42. | |
trouble. Rescuers arrived within minutes and spotted the daughter | :02:43. | :02:46. | |
already submerged in the water. She was very hypothermic and frightened | :02:47. | :02:52. | |
and concerned for her mother. We didn't have any news by then | :02:53. | :02:58. | |
other than when she was located in the air and sea rescue said it was | :02:59. | :03:03. | |
deemed better that they would recover the needy from the water. | :03:04. | :03:12. | |
This area is known as treacherous because when a person is cut off, | :03:13. | :03:18. | |
the force of water is so strong that there would be no possibility of | :03:19. | :03:22. | |
trying to swim for sure. 40-year-old Emma Hackett survived but her | :03:23. | :03:27. | |
68-year-old mother Susan Hackett could not be saved. Five of the dog | :03:28. | :03:34. | |
also drowned. Tonight rescue teams are warning people to check the tide | :03:35. | :03:40. | |
times before heading out. As you can see, we are blessed with a lovely | :03:41. | :03:44. | |
coastline but it presents a danger. People need to leave enough time to | :03:45. | :03:48. | |
get back from wherever they walk to but ultimately, if they feel they | :03:49. | :03:53. | |
are in any danger they need to call the coastguard immediately. The | :03:54. | :04:01. | |
winning -- the warning signs are there. It seems to be worse since | :04:02. | :04:08. | |
the last storms we had because I think the sand is a not flatter. The | :04:09. | :04:14. | |
tide was a long way out yesterday. You have to worry about the tide | :04:15. | :04:18. | |
because it can come in suddenly. Having said that, it goes out so far | :04:19. | :04:22. | |
you wouldn't have thought it would be that quick but it is obviously | :04:23. | :04:26. | |
dangerous. This stretch of coastline has long been notorious for failures | :04:27. | :04:35. | |
and now it proves it still has the ability to claim lives. There were a | :04:36. | :04:39. | |
couple of other incidents where emergency crews were sent out | :04:40. | :04:45. | |
because they had become stranded, one in the mumbles and another near | :04:46. | :04:51. | |
Penarth on Sully Island. The RNLI said we are experiencing | :04:52. | :04:56. | |
particularly high spring tides at the moment and we need to make sure | :04:57. | :05:00. | |
we leave enough time to get back because as we can see here, | :05:01. | :05:04. | |
especially on the sandbanks, the tide can come in much quicker than | :05:05. | :05:08. | |
you think. A mine manager facing four counts of | :05:09. | :05:10. | |
manslaughter through gross negligence - failed to comply with | :05:11. | :05:14. | |
safety regulations - a jury has heard. The prosecution says Malcolm | :05:15. | :05:17. | |
Fyfield took "staggering risks" at the Gleision Mine in the Swansea | :05:18. | :05:20. | |
valley, which flooded after a wall was breached in 2011. He denies the | :05:21. | :05:26. | |
charges. Cemlyn Davies was in court. A simple but catastrophic mistake | :05:27. | :05:31. | |
that should never have been made. That's how the prosecution described | :05:32. | :05:34. | |
what happened at Gleision on the day four colliers lost their lives. | :05:35. | :05:39. | |
Garry Jenkins, Philip Hill, David Powell and Charles Breslin all | :05:40. | :05:42. | |
drowned after 650,000 gallons of water poured into the section of the | :05:43. | :05:51. | |
mine where they working. The men were trying to break through to a | :05:52. | :05:55. | |
disused part of the mine and the jury heard Malcolm Fyfield had been | :05:56. | :05:58. | |
repeatedly warned about the presence of water in that area. But according | :05:59. | :06:03. | |
to the prosecution "Mr Fyfield decided to go his own way. He must | :06:04. | :06:08. | |
have thought he could breach the old workings without causing danger to | :06:09. | :06:16. | |
anyone", the barrister said. The jury heard Mr Fyfield told | :06:17. | :06:19. | |
police he'd gone into the old central workings the day before the | :06:20. | :06:23. | |
disaster and found there was no body of standing water there. It's the | :06:24. | :06:25. | |
prosecution's case that Malcolm Fyfield had misjudged where the | :06:26. | :06:30. | |
water was. The jury was shown these industry regulations which clearly | :06:31. | :06:33. | |
state "it's the mine manager's duty to prevent any inrush". Mr Fyfield | :06:34. | :06:39. | |
should have notified the mine inspector of his intention to break | :06:40. | :06:43. | |
through to the old workings even if he didn't think there was water | :06:44. | :06:49. | |
there. The prosecution say he failed to comply with that regulation. | :06:50. | :06:52. | |
Concluding his opening statement, Gregg Taylor QC said, "The mine | :06:53. | :06:56. | |
manager is there to make the correct calculations and men's lives depend | :06:57. | :06:57. | |
on him doing so. This afternoon the first witness was | :06:58. | :07:11. | |

confirmed Mr Fyfield was a meticulous mine manager who always | :07:15. | :07:16. | |

authority care however a third of the boys and 44% of the girls in | :15:17. | :15:21. | |
youth custody have been our are still in care. Last year the Justice | :15:22. | :15:24. | |
Committee said that the youth justice system was failing this | :15:25. | :15:27. | |
group of people. I still believe the cohorts between | :15:28. | :15:33. | |
18 and 21 when they leave care are not being provided for. There is a | :15:34. | :15:38. | |

controls for clinics carrying out cosmetic piercing, acupuncture and | :20:07. | :20:09. | |
electrolysis. A new hygiene rating system is also being introduced at | :20:10. | :20:11. | |
tattoo parlours which will be similar to the scores on the doors | :20:12. | :20:16. | |
food hygiene rating. The risks of not getting done properly could mean | :20:17. | :20:25. | |
hepatitis, HIV and scarring. Tattooing done properly is an | :20:26. | :20:29. | |
expensive feature, the artwork is expensive, the ink is expensive and | :20:30. | :20:33. | |
it takes time to do properly which is why we are seeing a rise of the | :20:34. | :20:39. | |
cheap tattooists who can access equipment on the Internet and setup | :20:40. | :20:40. | |
