Tonight, the First Minister and The Health Minister have apologised | :02:18. | :02:23. | |
to families of patients and have ordered a Wales wide review of | :02:24. | :02:27. | |
Here's our health correspondent Owain Clarke. | :02:28. | :02:39. | |
The government cannot hide behind this report. All of these families | :02:40. | :02:48. | |
claim the Swansea health board had left them and their loved ones down. | :02:49. | :02:50. | |
They each other story about that care. They saw today one of the most | :02:51. | :02:58. | |
damning reports on care standards in Wales in recent years. They have not | :02:59. | :03:04. | |
taken any consideration of surgery. With them was Gareth Williams from | :03:05. | :03:08. | |
Porthcawl. An investigation last summer revealed his mother suffered | :03:09. | :03:13. | |
serious neglect during the last two years of her life. This video by the | :03:14. | :03:18. | |
family shows medicine scattered around the hospital bed. She had | :03:19. | :03:23. | |
been sedated against their wishes. The health board ultimately | :03:24. | :03:27. | |
apologised but ever since Gareth Williams has led a campaign to | :03:28. | :03:32. | |
expose other examples of poor care which party prompted the | :03:33. | :03:34. | |
investigation that was published today. He told us apologies aren't | :03:35. | :03:40. | |
enough. It is a public disgrace. There must be a public enquiry and | :03:41. | :03:44. | |
in the interim while we wait for a government in denial to grant one | :03:45. | :03:48. | |
they must be the removal of the Chief Executive and the management | :03:49. | :03:53. | |
team. The team of experts brought in by the Welsh Government spent months | :03:54. | :03:57. | |
talking to staff, patients and their families. Some of what they had | :03:58. | :04:04. | |
caused them real concern. Some patients were not getting the water | :04:05. | :04:08. | |
they needed, the food they needed, the medication they needed. These | :04:09. | :04:13. | |
were largely exceptional events but they should not happen. There were | :04:14. | :04:19. | |
several other feelings for example older patients in bed instructed to | :04:20. | :04:24. | |
go to tile at Waverley. They were dismayed that staff tolerated | :04:25. | :04:30. | |
hazardous practices which meant medicines were recorded as given | :04:31. | :04:33. | |
when sometimes they were not. It notes a failure of adequate staffing | :04:34. | :04:38. | |
20 47 and managers being distracted trying to cut costs at the expense | :04:39. | :04:44. | |
of quality and patient safety. Concerns were also raised about the | :04:45. | :04:48. | |
effectiveness of health care Inspectorate Wales in not picking up | :04:49. | :04:55. | |
some of the issues. All in all there are 18 specific recommendations for | :04:56. | :04:58. | |
the local health board and Welsh Government. Both say they accept | :04:59. | :05:03. | |
them all and this afternoon the Health Minister went further | :05:04. | :05:06. | |
announcing spot checks would be held within months at all Welsh | :05:07. | :05:12. | |
hospitals. As from today, I will give health boards and NHS trusts in | :05:13. | :05:17. | |
Wales a four-week period to absorb the findings of the report and | :05:18. | :05:20. | |
satisfy themselves that such departures from basic professional | :05:21. | :05:25. | |
standards are not present in their organisations. Immediately after | :05:26. | :05:31. | |
that, each district general hospital in Wales will be visited by a small | :05:32. | :05:37. | |
group of independent experts. But others argue this report findings | :05:38. | :05:41. | |
contains ominous echoes of the mid Staffordshire neglects candle in | :05:42. | :05:48. | |
England. He may not think that this is a mid-Staffs crisis and I don't | :05:49. | :05:51. | |
think the report authors can conclude that given the narrow focus | :05:52. | :05:56. | |
of their work. But I have this one final thing to say. If it looks | :05:57. | :06:03. | |
like, smells like and moves like a mid-Staffs crisis, it's time for a | :06:04. | :06:07. | |
major enquiry into care standards in the Welsh NHS. The elderly care | :06:08. | :06:12. | |
expert who led the review begs to differ. There is a huge difference | :06:13. | :06:17. | |
between the catastrophic organisational failure that happened | :06:18. | :06:21. | |
in mid-Staffs and the situation here. In mid-Staffs the Danish | :06:22. | :06:26. | |
health service had no idea that it had a problem at all. Here, by the | :06:27. | :06:35. | |
time the Minister was aware, you had asked us to review it and the health | :06:36. | :06:42. | |
board knew there were issues. The report does give some cause to be | :06:43. | :06:47. | |
hopeful claiming that with immediate effort things can be turned around. | :06:48. | :06:55. | |
That with ten nurses suspended from duty and police investigation is | :06:56. | :06:57. | |
ongoing these families are still concerned. They consider themselves | :06:58. | :07:00. | |
to be victims of the care system that didn't care enough. | :07:01. | :07:05. | |
Let's talk to the chair of the Health Board, | :07:06. | :07:07. | |
One patient told the review team "I am in hell". | :07:08. | :07:11. | |
I was ashamed and I told the Minister that. We accept all the | :07:12. | :07:26. | |
recommendations of the report and we apologise for any poor care that we | :07:27. | :07:31. | |
have provided in recent years. Nurses claiming that patients would | :07:32. | :07:39. | |
not their responsibility. Management not doing much about complaints from | :07:40. | :07:43. | |
families. It does not get much worse does it? Absolutely unacceptable. We | :07:44. | :07:50. | |
have been talking today with members of staff here and we will do that | :07:51. | :07:53. | |
over the coming weeks. We have said clearly that that type of care is | :07:54. | :07:59. | |
unacceptable. If we become aware of it people should be made -- making | :08:00. | :08:08. | |
us aware of it. When I arrived here tonight I was met by a dozen | :08:09. | :08:11. | |
families who told me they were making complaints going back years. | :08:12. | :08:16. | |
Nothing was done and nobody was listened to. They are calling for | :08:17. | :08:21. | |
resignations tonight. Why has nobody resigned? The report said clearly | :08:22. | :08:27. | |
they have confidence in the leadership of the board to put right | :08:28. | :08:31. | |
what has been happening over many years. This has happened on | :08:32. | :08:39. | |
Baywatch. We are not a mid Staffordshire. The difference | :08:40. | :08:46. | |
between... We started to take action. You did not recognise the | :08:47. | :08:53. | |
deficiencies early. The families were telling the hospital this years | :08:54. | :09:04. | |
ago and nobody listened. I took up this office in January 2013. Serious | :09:05. | :09:11. | |
concerns came to light in March and we have moved very quickly. We have | :09:12. | :09:16. | |
changed the decision-making and we have challenged the bad practice. We | :09:17. | :09:23. | |
have changed management locally here. We are making wider changes | :09:24. | :09:26. | |
across the health board. So heads will roll? People have not performed | :09:27. | :09:38. | |
and they are no longer in position and we have appointed new people. We | :09:39. | :09:42. | |
have strengthened the local management and we are strengthening | :09:43. | :09:46. | |
the management in the health board. We accept the changes have not been | :09:47. | :09:51. | |
fast enough but that is why we welcomed the report. We think we can | :09:52. | :09:54. | |
produce something very special here and across the health board. | :09:55. | :10:21. | |
Many will hope this trial is a line under a row that goes back two years | :10:22. | :10:24. | |
or more to a decision by the Betsy Cadwallader health board to move the | :10:25. | :10:31. | |
treatment of seriously ill babies to a hospital over the border. John | :10:32. | :10:35. | |
Hewitt, you campaigned against that initial decision and you campaigned | :10:36. | :10:40. | |
for the regional centre to be tastier. Wrexham were hoping they | :10:41. | :10:45. | |
would get it. That is disappointment as well from some people who are | :10:46. | :10:49. | |
concerned that the most seriously ill babies will still be treated | :10:50. | :10:55. | |
over the border. I say to the people in Wrexham, two years ago there was | :10:56. | :11:00. | |
not going to be any investment in a new mental -- neonatal service in | :11:01. | :11:04. | |
North Wales. They were going to ship everybody over to London. To your | :11:05. | :11:15. | |
second point, I would say there has always been a case for seriously ill | :11:16. | :11:19. | |
babies. And you have benefited from that in your family? We have. Our | :11:20. | :11:27. | |
baby was born here in this unit and she would not be alive today if it | :11:28. | :11:31. | |
wasn't for the fantastic perfectionism of the staff here. The | :11:32. | :11:35. | |
first minister says this decision comes with risks and challenges. | :11:36. | :11:40. | |
There are concerns that the health board will not be able to deliver on | :11:41. | :11:49. | |
this side of it. They will have to. That is quite clear. There is a | :11:50. | :11:52. | |
change of the higher management now in Betsy Cadwallader and as a result | :11:53. | :11:58. | |
there needs to be a higher change so they're going to have two get their | :11:59. | :12:03. | |

water extraction. It was at that point they said no it has been | :18:03. | :18:15. | |
allocated to another company. The National Trust is adapting its | :18:16. | :18:18. | |

this morning every health board boss in Wales was ordered to look at this | :23:57. | :24:02. | |
report and measure their services for elderly patients against it. | :24:03. | :24:08. | |
After a month, inspectors come experts will go into every hospital | :24:09. | :24:12. | |
in Wales and judge whether or not elderly care is up to scratch. The | :24:13. | :24:18. | |

Apologies to campaigners are hollow without resignations but I was | :24:34. | :24:37. | |

came to the view that if heads had rolled, that would have left a power | :24:45. | :24:50. | |
void at the top of the health board at the very time improvements needed | :24:51. | :24:55. | |
to be made. The view was that would not have been good for patients. | :24:56. | :25:01. | |
