with her six-year-old daughter and her 11-month-old son. | :01:18. | :01:20. | |

The fire broke out just after 1:30am this morning. | :01:25. | :01:26. | |

Neighbours using ladders had to rescue four-year-old Jac | :01:30. | :01:33. | |
While they were taken to hospital for treatment of the effects | :01:34. | :01:46. | |
of breathing in smoke, he sadly died here at the scene. | :01:47. | :01:48. | |
Too upset to speak, Jac's mother wrote her tribute calling him her | :01:49. | :01:51. | |
The note continues, "You're my life, my world, my everything. | :01:52. | :01:55. | |
"Go ride your train up to heaven and wait for me." | :01:56. | :01:57. | |
Always talkative, had everything going for him. | :01:58. | :02:03. | |
But a joint investigation by the police and Fire | :02:04. | :02:14. | |
At this point in time, the fire investigation is ongoing. | :02:15. | :02:19. | |
Until we get the report from the officers we | :02:20. | :02:21. | |
But we will be a reassuring the statement as soon as we know | :02:22. | :02:37. | |

It's believed the house was fitted with smoke detectors and they may | :02:40. | :02:42. | |

Tonight, one of her friends told us she was very popular at school. | :03:01. | :03:12. | |
She was a bubbly girl. She never kept herself to herself. She got | :03:13. | :03:20. | |
along with people. Had plenty of mates in school. Nothing out of the | :03:21. | :03:27. | |
ordinary, just a normal teenage girl. As far as parting ways at the | :03:28. | :03:34. | |
end of school, that was the normal thing, really. To find out today now | :03:35. | :03:37. | |
everything that has gone on since we have left school is gobsmacking, | :03:38. | :03:43. | |
really. I had to double-check that it was the same person. | :03:44. | :03:44. | |
A fire involving rubber panels, which caused disruption in Cardiff | :03:45. | :03:46. | |

Plaid Cymru has asked the Welsh Government to explain why | :03:56. | :04:02. | |
it believes that holding the 2026 Commonwealth Games in Wales | :04:03. | :04:05. | |

The ambulance service in Wales has turned a corner and is "getting | :04:21. | :04:31. | |
better," but continues to face big challenges as demand increases. | :04:32. | :04:33. | |
That's according to Health Secretary, Vaughan Gething. | :04:34. | :04:35. | |
The latest figures for June show the service responded | :04:36. | :04:37. | |
within eight minutes to over 77% of life-threatening 999 calls - | :04:38. | :04:40. | |
its best performance since new targets were introduced last | :04:41. | :04:42. | |

And that helps when a call like this comes in. | :04:58. | :05:01. | |
I've joined him in a rapid response car that can easily weave in and out | :05:02. | :05:12. | |

She was worried he'd knocked his head. | :05:19. | :05:25. | |
After the checks, all agree Tom doesn't need to go to hospital. | :05:26. | :05:28. | |
I didn't call for back-up, so the emergency ambulance that | :05:29. | :05:31. | |
might have come is now at a higher priority call. | :05:32. | :05:35. | |
But had this happened last year, an ambulance or even several may | :05:36. | :05:38. | |
well have been sent in an effort to arrive within eight minutes. | :05:39. | :05:42. | |
But now that target's been scrapped for all but immediately | :05:43. | :05:44. | |
The Ambulance Service Director of Operations explains why. | :05:45. | :05:50. | |
We could have invested tens of millions of pounds in hitting | :05:51. | :05:53. | |
But it wouldn't have made any difference to patients | :05:54. | :05:57. | |
because we know that around 20% of our previous red patients weren't | :05:58. | :06:01. | |
Our system previously categorised calls as potentially | :06:02. | :06:06. | |
life-threatening and that was a very vague term. | :06:07. | :06:10. | |
Since the new system came into force in October, far fewer 999 calls have | :06:11. | :06:13. | |
been classified as needing an eight minute response. | :06:14. | :06:17. | |
In June last year under the old system, the target applied | :06:18. | :06:19. | |
In June this year it was for just over 1,500 incidents. | :06:20. | :06:26. | |
For all other calls the Ambulance Service is given more | :06:27. | :06:29. | |
The smaller number of calls that you have to get | :06:30. | :06:33. | |
to within eight minutes, theoretically, the easier | :06:34. | :06:35. | |
However, it's really, really important that we do get | :06:36. | :06:42. | |
What we've been able to demonstrate it we can get to a lot more | :06:43. | :06:48. | |
of the calls for those people that have immediately life-threatening | :06:49. | :06:50. | |
conditions than we were ever able to before. | :06:51. | :06:54. | |
Critics have previously described all of this | :06:55. | :06:56. | |
as a dangerous experiment - an attempt by the Welsh Government | :06:57. | :06:58. | |
But supporters claim the new system gives far more information | :06:59. | :07:03. | |
not just about speed, but also about quality of care. | :07:04. | :07:12. | |
For example, between January and March, ambulance crews spent | :07:13. | :07:14. | |
22,000 hours stuck outside A departments waiting | :07:15. | :07:16. | |
And there are differences across the country in how many 999 | :07:17. | :07:20. | |
calls end up with patients being treated in hospital. | :07:21. | :07:23. | |
But, overall, the Health Secretary argues that the Ambulance Service, | :07:24. | :07:26. | |
so often in the news for the wrong reasons, is now firmly | :07:27. | :07:29. | |
I'm really encouraged by the fact there's been | :07:30. | :07:33. | |
lots of honesty in the data about what the Ambulance Service | :07:34. | :07:36. | |
does well and what it doesn't do so well. | :07:37. | :07:38. | |
And also to see quality and improvement acting | :07:39. | :07:40. | |
Not just to say, "Here challenges, don't blame us." | :07:41. | :07:46. | |
But, "Here are challenges and this is what we will do about them." | :07:47. | :07:53. | |
Back in the rapid response car, we hear a child has been injured | :07:54. | :07:56. | |
This is an eight minute call and we pull out | :07:57. | :07:59. | |
But before we arrive it turns out to be a false alarm. | :08:00. | :08:04. | |
For some emergencies, speed is critical. | :08:05. | :08:06. | |
But according to this paramedic, the trick is to know when. | :08:07. | :08:11. | |
The cost of covering overtime payments for consultants at Welsh | :08:12. | :08:15. | |
hospitals has gone up by over 60% in the last three years. | :08:16. | :08:19. | |
shows that the total bill went from more than 5 million | :08:20. | :08:30. | |
pounds in 2013 to 2014 to more than ?8 million | :08:31. | :08:32. | |
The Welsh Government says it's working to recruit more consultants, | :08:33. | :08:36. | |
and is addressing specialist shortages. | :08:37. | :08:37. | |
It's a large amount of money, let's be honest about this. | :08:38. | :08:40. | |
But what these figures do reflect is the lack of consultants able | :08:41. | :08:43. | |
We know, for instance, that in May 2016 just gone, | :08:44. | :08:52. | |
the demand for new referrals to outpatients and clinics has gone | :08:53. | :08:54. | |
up by about 4% compared to just one year ago. | :08:55. | :09:01. | |
The number of young people trespassing on the railways in Wales | :09:02. | :09:03. | |
has increased by almost 20% percent over the past year | :09:04. | :09:06. | |
according to British Transport Police. | :09:07. | :09:09. | |
They say the problem is at its worst in the summer months, | :09:10. | :09:12. | |
when days are longer and children are off school. | :09:13. | :09:14. | |
These shocking pictures reveal a number of close calls for people | :09:15. | :09:22. | |
trespassing on the tracks, including the following | :09:23. | :09:24. | |
They were lucky - Victoria Swift wasn't. | :09:25. | :09:35. | |
The 14-year-old from Rhosllanerchrugog, near Wrexham, | :09:36. | :09:39. | |
died after being hit by a train near a pedestrian rail crossing | :09:40. | :09:41. | |
Her family can't understand why anyone would risk their | :09:42. | :09:45. | |
When we lost Victoria it took a long, long time to just... | :09:46. | :09:51. | |
Well, I wouldn't even say we still normal today. | :09:52. | :09:55. | |

Victoria Swift died in 2007, but it wasn't until last | :10:26. | :10:28. | |
year that this crossing was finally closed. | :10:29. | :10:30. | |
This will protect young people living in this area, | :10:31. | :10:32. | |
but there are fears the message isn't getting across everywhere. | :10:33. | :10:34. | |
British Transport Police say there's been a 19% rise in trespass | :10:35. | :10:37. | |
incidents in the summer months with longer days and warmer | :10:38. | :10:39. | |
There were 128 reported cases of trespass last summer | :10:40. | :10:42. | |
in Wales alone, 20 more than the previous year. | :10:43. | :10:44. | |
Make sure that your child knows the dangers of going | :10:45. | :10:47. | |
That they understand the dangers of going onto railway tracks. | :10:48. | :10:53. | |
And if, for example, they're playing nearby | :10:54. | :10:55. | |
and a football went on to the lines by accident, would they know | :10:56. | :10:58. | |
Would they go running on the tracks to retrieve their football, | :10:59. | :11:03. | |
or would they know to stand back and to contact British | :11:04. | :11:05. | |
