Jenson Barnett from Newtown died in 2013 from brain injuries,

two days after a failed attempt to deliver him with forceps.

The boss of the health trust has apologised unreservedly

:00:59.:01:00.

Little Jenson Barnett should have been nearly four years old now.

:01:01.:01:09.

But he died just two days after he was born

:01:10.:01:11.

Staff tried to deliver him with forceps but they slipped

:01:12.:01:16.

off his head causing injuries to his brain.

:01:17.:01:18.

He was later born by emergency caesarian section but it was

:01:19.:01:22.

We spent that night with him but obviously he had

:01:23.:01:25.

There were six or seven, maybe even more, doctors around him.

:01:26.:01:31.

We spent the night with him and then the day he got moved to a different

:01:32.:01:42.

unit with lots more tubes again but yes, it was not nice.

:01:43.:01:51.

Jenson was one of a number of babies to die at Shrewsbury

:01:52.:01:54.

and Telford NHS Trust over the past eight years.

:01:55.:01:56.

Seven of those deaths, including Jenson's,

:01:57.:01:59.

Kate, who's since had a daughter Isla, says despite an apology

:02:00.:02:05.

and assurances that lessons would be learned she's angry that

:02:06.:02:08.

similar tragedies continued at the trust.

At the time they said they were going to do stuff about

:02:12.:02:16.

it and things had changed, but obviously they have not.

Seeing all this on the news has really made me quite

The Trust, which also provides services

for patients across the border in mid Wales delivers around 4,700

babies each year - around 224 of them are from the Powys area.

Tonight its chief executive has apologised unreservedly to the

He said, in recent years independent external reviews have repeatedly

shown the Trust is progressing and learning lessons but that it

England's Health Secretary Jeremy Hunt has asked for a review

into the deaths and other incidents at the trust

and for the families affected to be kept informed.

The Barnett's local MP has welcomed the investigation.

One needs to have an investigation into what happened.

There has to be full transparency in order

There has to be the most fulsome of apologies to any

families thathave been affected and had such

Kate Barnett and her husband say that Jenson will always be a big

part of their family and hope other families will be

You go into hospital to have a baby, you do not expect to come out

I don't think anything will bring Jenson back

but our family and friends, are strong together as well.

Many policies aimed at reducing poverty in Wales

That's the damning finding of a major new study

:03:55.:03:57.

into the poorest place in the country -

Experts spent six months analysing the estate.

Our economics correspondent, Sarah Dickins, has

Lansbury Park sits almost in the shadow of Caerphilly's

:04:07.:04:13.

famous castle but it is a world away from its grandeur.

This is the most deprived community in Wales and although proud

:04:17.:04:20.

and tight knit, it has complex problems.

:04:21.:04:24.

It feels like one that is almost hemmed in, it's one that is almost

:04:25.:04:28.

As a nation we are creating economic policies which are dislocated

:04:29.:04:33.

from people who live in communities like Lansbury.

:04:34.:04:37.

We need to talk to them, engage them rather than leave them behind.

:04:38.:04:42.

With the children in school a group of mothers start their work-out.

:04:43.:04:45.

Lorna and Heid were brought up in Lansbury Park and are still

:04:46.:04:47.

It does not mean that is what is going on in the community.

:04:48.:04:56.

Although Lansbury Park is close to jobs in Cardiff and the M4,

:04:57.:05:04.

I'm told it is hard for young people to get a job.

:05:05.:05:07.

I think there are jobs out there and I think it does not help

:05:08.:05:11.

when people would like to go for a job that they think, it is not

:05:12.:05:16.

worth me trying because I haven't got the qualifications,

:05:17.:05:21.

Two thirds of lone parents are not employed, that's 50% higher

:05:22.:05:27.

Men living on Lansbury Park are more than twice as likely to be

:05:28.:05:33.

You can see the challenges, more than half of the adults

:05:34.:05:39.

on the estate have no qualifications and half have no car.

:05:40.:05:44.

Caerphilly Council owns the social housing, not a housing association.

:05:45.:05:47.

It is making the houses warmer and drier as part of a ?10 million

:05:48.:05:51.

It is also trying to set up work experience for residents

:05:52.:05:56.

It knows it needs to do things very differently and argues that

:05:57.:06:03.

It is about confidence, it is about giving those people

:06:04.:06:07.

the confidence and the self belief that they are worth it.

:06:08.:06:11.

It means working with them to make them believe

:06:12.:06:14.

People say you are not good enough, you believe you are not good enough,

:06:15.:06:27.

There is a real mix of problems here and certainly

:06:28.:06:31.

I have been talking to people at community events,

:06:32.:06:35.

and to community workers here as well and what they tell

:06:36.:06:38.

me is that the group of people who are hardest

:06:39.:06:41.

One solution in the report is that there should be more

:06:42.:06:46.

day-to-day or even mundane jobs here on the estate,

:06:47.:06:49.

for instance, repairing things, caring for people

:06:50.:06:52.

That way there could be jobs here that people would not need

:06:53.:06:57.

too many qualifications for and they would not

:06:58.:06:59.

As the Welsh Government determines its future

anti-poverty policies, the message from Lansbury Park will

The sister of a woman unlawfully killed at a hostel in Argoed has

spoken about how she discovered she'd been murdered on social media.

Cerys Yemm was attacked by Matthew Williams in November 2014.

Police officers stunned him with a Taser but he later died.

I had seen a post obviously that there had been a murder in Argoed.

Then I had people messaging me via Facebook message saying

please don't tell me it's true, it's not your sister.

I had to drive home from Cardiff then,

Mum was crying, couldn't tell me, couldn't speak on the phone.

Gwent Police say there was a delay in telling Cerys Yemm's

family about her death, because they wanted her mother to be

:07:55.:07:56.

told by an officer who'd dealt with her previously.

The former head of a Swansea homeless charity has

:08:02.:08:03.

admitted forging invoices for ?1.3 million into his own bank account

:08:04.:08:08.

Robert Davies was working for Cyrenians Cymru

when he was arrested in December 2014.

The charity later went into administration.

Davies was remanded in custody, to be sentenced later this month.

Today members there were planning a simulated rescue on Tryfan,

one of the toughest peaks in Snowdonia.

Together the six mountain rescue teams in Wales have had 80

As volunteers, we are leaving our homes, work and families to carry

:10:03.:10:08.

out rescues and it is very frustrating when we find people who,

:10:09.:10:14.

it is almost a self-inflicted injury by going out ill-prepared.

It is a similar story in South and Mid Wales.

The Central Beacons team has had an unprecedented 51 calls

since New Year, they know Easter

We strive to provide a professional service

but we are volunteers and potentially with the increasing

trend in calls for help, there is potential for a drain

Without exception, mountain rescuers love the terrain in which they work

but they constantly warn that beautiful as they are,

the mountains will come back and bite you.
