from Leicester. Until today. The last time the Queen visited was in

2012 at the start of a Jubilee Tower. Today, just like then, people

were delighted to see her. -- we start off jubilee tour.

Early morning and a sea of faces in Leicester.

Thousands lined the streets, eager to catch a glimpse

of the Queen as she arrived for the annual Maundy

We were here at 20 past eight this morning.

She looked fantastic and she was waving to everyone as well.

It was lovely to see her and nice to see her in Leicester.

At Leicester Cathedral, the Yeoman of the Guard patrol the grounds,

ready for Her Majesty to carry out this most ancient of royal duties.

The Maundy ceremony dates back centuries and is an important annual

:02:45.:02:47.

It has been hosted at every cathedral across the country,

:02:48.:02:50.

The Queen and Duke of Edinburgh were welcomed at the cathedral

:02:51.:02:59.

by the Dean of Leicester and the Bishop, delighted to be

:03:00.:03:02.

The Maundy service happens each year, but this is the last

:03:03.:03:09.

of the English cathedrals the Queen has been to and she was very aware

:03:10.:03:15.

of that, very conscious that we had waited a long time.

:03:16.:03:18.

In her 91st year, the Queen presented 91 men and 91 women

:03:19.:03:20.

with specially minted money in two leather purses.

:03:21.:03:25.

It is a ceremony that, over the years, has changed.

The monarch used to wash the feet of pensioners.

:03:28.:03:29.

Now, Her Majesty prefers to shake hands.

One of the lucky few receiving the purses is Marilyn, who,

like all of those honoured, has spent her life helping others.

There are not that many people who get these.

I am just so lucky to be one of them.

With the ceremony over, the Royals posed for photographs

with local schoolchildren at the end of a service that leaves

all cathedrals now visited and one that many here in Leicester

After the service, just to give you a bit more of a flavour of her

visit, the Queen headed to Saint Martin's house just behind me, where

she enjoyed a three course meal of locally produced food, including

trout from a local river, before being taken home. That left every

Next tonight, chaos, collusion and a lack of professional

curiousity are just some of the findings of a serious case

:05:03.:05:06.

review into the death of a seven year old girl from Nottingham.

:05:07.:05:09.

Shanay Walker died in 2014 from a brain injury and was found

:05:10.:05:12.

Her aunt and grandmother were both jailed for cruelty.

:05:13.:05:16.

In a moment, we'll be hearing from the chair

:05:17.:05:18.

of the city's Safeguarding Board, but first, Quentin Rayner has been

:05:19.:05:21.

In assessment videos, Shanay Walker appeared

:05:22.:05:28.

to have a good relationship with her aunt, Kay-Ann Morris,

:05:29.:05:31.

Her guardian was in fact harsh, cruel and abusive to her

:05:32.:05:35.

Morris was cleared of murder but was jailed for cruelty,

:05:36.:05:38.

along with Shanay's grandmother Juanila Smikle.

:05:39.:05:44.

The review found that concerns were raised about bruising to Shanay

:05:45.:05:47.

And there was a lack of professional cure the city. Safeguarding and

:05:48.:05:56.

Rangers were described as chaotic at Southglade primary school. A number

:05:57.:06:00.

of teachers raised concerns about abuse and injuries but they were not

:06:01.:06:03.

acted upon. One safeguarding form went missing and after Shanay

:06:04.:06:08.

Walker's death, it was rewritten by a head teacher and teaching

:06:09.:06:15.

assisting. The serious case review made another of other conclusions.

There was little case discussion and reflection on a melting new

Can I start by asking you what collectively went wrong

among the professionals who dealt with this case?

Before I answer your question, can I just say that on behalf

of the Safeguarding Board, I would like to express our sincere

condolences to the family and friends of Shanay.

What went wrong, in essence, there was a great deal of difficulty

:07:33.:07:41.

around information sharing and a lack of

:07:42.:07:43.

Isn't that key to the role of people working in this field?

:07:44.:07:52.

It is extremely key, but on this occasion,

:07:53.:07:55.

There needed to be more professional curiosity.

:07:56.:08:01.

It is, in this case, a fact that the aunt who was caring

:08:02.:08:09.

for Shanay at the time was deceitful and she duped the professionals.

:08:10.:08:12.

Information sharing, deficiencies in people

:08:13.:08:13.

talking to each other between departments and agencies...

:08:14.:08:15.

This seems to happen time and time again.

:08:16.:08:19.

It always seems to be highlighted in serious case reviews.

:08:20.:08:21.

It is highlighted in many serious case reviews but it

:08:22.:08:24.

does not surprise me, because where you have cases

:08:25.:08:26.

being managed by many agencies, I think there is a likelihood that

:08:27.:08:29.

I accept that and we need to improve on that.

:08:30.:08:34.

And the Safeguarding Board has a role to do in that

:08:35.:08:37.

Because that also seems to happen quite rarely in similar questions.

:08:38.:08:42.

At the moment, there is nobody that has been held to account

:08:43.:08:45.

You will note from the inquest that the coroner could not find any

:08:46.:08:51.

one individual culpable for these matters.

:08:52.:08:53.

And finally, a word about the school.

What was your assessment of the safeguarding arrangements

Safeguarding arrangements were chaotic, and that is what we have

We are working with the benefit of hindsight.

A man's been jailed for seven years for raping a woman

years ago are trying to track down an East Midlands family who could

The little girl went missing from a beach just

Police now say they want to speak again

and are now believed to be living in Nottinghamshire.

The family of Cheryl Grimmer are still searching for the truth

The youngest of four, her family emigrated from Bristol

Two years later, on a day at the beach with her mum

Last month, nearly 50 years on, a breakthrough in the case
