Good evening and welcome to BBC Look North. | :00:00. | :00:00.
Failings in care, leadership and patient safety - | :00:00. | :00:16.
the damning report on the health of Lincolnshire's hospitals. | :00:17. | :00:21.
The trust hasn't gotten a focus on patient safety and managing the | :00:22. | :00:25.
risks patients present. Claims of a care crisis as another | :00:26. | :00:28.
home for the elderly A man's jailed after spending four | :00:29. | :00:31.
years trying lie his way Hull hosts an explosive performance | :00:32. | :00:36.
on a floating stage as part And a bit of brightness today but we | :00:37. | :00:51.
had an funny bit of a change overnight. The forecast from myself | :00:52. | :00:52.
shortly. A catalogue of failings in care, | :00:53. | :01:00.
leadership and safety means government experts are again | :01:01. | :01:03.
going into Lincolnshire's hospitals Four years after inspectors | :01:04. | :01:05.
first placed the trust responsible in special measures | :01:06. | :01:12.
they are back in again. Bosses have apologised and said | :01:13. | :01:20.
things are already improving. I'll be speaking to the man | :01:21. | :01:23.
in charge shortly, but first our health correspondent | :01:24. | :01:25.
Vicky Johnson is here. This is not a good report | :01:26. | :01:27.
for hospital managers? This is another damning report | :01:28. | :01:31.
whichever way you look at it - showing a catalogue of failings | :01:32. | :01:34.
almost across the board Unsafe, poorly led by managers | :01:35. | :01:36.
and 'inadequate' are the fundamental A failure to refer cancer patients | :01:37. | :01:42.
to treatment on time. Thousands of outpatients | :01:43. | :01:48.
appointments long overdue - And A services at both Lincoln | :01:49. | :01:52.
and Boston rated as inadequate. When inspectors went on their rounds | :01:53. | :02:00.
in Lincolnshire's hospitals last autumn they found much | :02:01. | :02:02.
to criticise, including Some of the patient care that we saw | :02:03. | :02:18.
wasn't of the required standard, so for example, we saw some patients | :02:19. | :02:23.
that were not treated with compassion, we are dignity and | :02:24. | :02:27.
respect was compromised. We also found some concerns in the emergency | :02:28. | :02:29.
department. This is the latest in a series | :02:30. | :02:31.
of poor inspections. Four years ago it was put | :02:32. | :02:33.
into special measures Two years later it was taken | :02:34. | :02:36.
out of special measures Last October, inspectors | :02:37. | :02:39.
found more failings. have enough staff in Lincoln's A | :02:40. | :02:45.
and declared a "critical incident" And now confirmation that experts | :02:46. | :02:49.
have again been asked to step in. The sea QC report has made for | :02:50. | :03:05.
difficult reading for many. In particular, patients not receiving | :03:06. | :03:08.
medication on time when they are seriously ill as a big worry and | :03:09. | :03:12.
they think it is something that has been going on for some time. As a | :03:13. | :03:17.
doctor reading that report I was concerned by a number of aspects and | :03:18. | :03:22.
fully understand why special measures are required. That has been | :03:23. | :03:26.
the right decision but equally I would reassure the public that the | :03:27. | :03:29.
vast majority are receiving very good care. | :03:30. | :03:34.
The trust has always had problems recruiting enough staff. | :03:35. | :03:36.
Last year it spent more than ?26 million on agency | :03:37. | :03:38.
Managers fear their reputation as a failing trust could make | :03:39. | :03:42.
To be absolutely honest, that is my biggest year. I would sincerely ask | :03:43. | :03:53.
people not to be discouraged for this. They can come on secondment | :03:54. | :03:58.
and temporary contracts, detest us out. I think they will find we are | :03:59. | :04:00.
such a good team. The hospital teams certainly face | :04:01. | :04:04.
a testing few months as they battle to raise standards and come out | :04:05. | :04:07.
of special measures. A lot of the criticisms are broad - | :04:08. | :04:09.
poor leadership, staff bullying Inspectors looked at how often | :04:10. | :04:12.
people with bed sores But they found three patients who'd | :04:13. | :04:17.
waited between three and almost eight and a half hours | :04:18. | :04:27.
for their checks. The fourth patient had no record of | :04:28. | :04:34.
being checked at all. Staff told inspectors | :04:35. | :04:36.
they didn't have the time. But if there's no time | :04:37. | :04:38.
for these basic tasks - then that's very telling | :04:39. | :04:40.
about the organisation as a whole. We will be asking for your views on | :04:41. | :04:51.
your experiences at hospitals in Lincolnshire in just a moment. | :04:52. | :04:52.
Earlier, I spoke to the chief executive of the United | :04:53. | :04:55.
I asked him whether he accepted the report wasn't good enough. | :04:56. | :05:01.
We are very disappointed with this but of course, | :05:02. | :05:03.
as ever in these reports, lots of examples of good practice. | :05:04. | :05:12.
We recognise a number of scores, 12% are not adequate | :05:13. | :05:17.
If half are rated good, half are rated not good, | :05:18. | :05:23.
and if patients are not treated with dignity, low staff | :05:24. | :05:26.
satisfaction, overall the trust is inadequate. | :05:27. | :05:28.
Why should the people of Lincolnshire feel safe? | :05:29. | :05:30.
Nothing in the report says we are not safe. | :05:31. | :05:34.
What it identifies is a number of things that need to be put right. | :05:35. | :05:40.
When the CQC came in October that while a number of actions | :05:41. | :05:45.
we put immediately in place and we think if they came back today | :05:46. | :05:49.
they would see many of those issues already resolved. | :05:50. | :05:56.
Some of those things are long standing and one | :05:57. | :05:58.
of the questions is recruitment and the the reconfiguration | :05:59. | :06:01.
Those will take some time to resolve. | :06:02. | :06:03.
The trust keeps blaming staff shortages but the inspectors found | :06:04. | :06:06.
staffing was in line with the national average | :06:07. | :06:08.
The reality is while the overall numbers may be comparable | :06:09. | :06:13.
to larger hospitals, if you can imagine the ease | :06:14. | :06:15.
with which Nottingham can have all staff in one place, | :06:16. | :06:18.
we don't have that luxury in Lincolnshire, so those same | :06:19. | :06:20.
numbers of staff have to be spread relatively thinly across our | :06:21. | :06:23.
They also find hospitals are not properly equipped to deal with major | :06:24. | :06:40.
incidents with equipment out of date and not working. | :06:41. | :06:42.
Is that not basic Janet and John stuff you're not getting right? | :06:43. | :06:54.
I think at the time there were simple wrrors not | :06:55. | :06:56.
being addressed and it was more about administration. | :06:57. | :07:00.
Just to reassure you and others listening we carry out exercises | :07:01. | :07:05.
and we are rated highly so we are very ready for any major | :07:06. | :07:09.
How long until you are out of special measures? | :07:10. | :07:12.
The CQC came last October but it is not down to us. | :07:13. | :07:16.
We look forward to demonstrating the improvements we have made. | :07:17. | :07:18.
Very good to talk to you and we will follow the story. | :07:19. | :07:27.
What's been your experience of hospitals in Lincolnshire? | :07:28. | :07:33.
Do you agree with the inspectors that the hospitals are inadequate? | :07:34. | :07:36.
Perhaps you work there - what do you make of | :07:37. | :07:38.
What does the trust me to do to improve? The address is there. | :07:39. | :08:04.
Some of your experiences. That's before we finish at seven p.m.. | :08:05. | :08:09.
People living in the small village of Lutton in South Lincolnshire have | :08:10. | :08:11.
been paying tribute to local school teacher and church | :08:12. | :08:14.
She had been reported missing two weeks ago. | :08:15. | :08:19.
It's believed her body was found along with that of her husband's | :08:20. | :08:22.
The couple have not yet been formally identified. | :08:23. | :08:25.
This small Lincolnshire village has been left in shock by the discovery | :08:26. | :08:35.
of two bodies at this house down a quite country lane | :08:36. | :08:37.
It's a pleasant village and it's just so quiet here, | :08:38. | :08:43.
for something like this to happen to a family is just unbelievable. | :08:44. | :08:46.
One of the bodies is believed to be that of teacher Bernice Williams | :08:47. | :08:50.
who went missing more than two weeks ago. | :08:51. | :08:56.
It is understood she lived here but the bodies of a man | :08:57. | :08:59.
and woman were only found at the weekend. | :09:00. | :09:03.
It is believed the other body is that of her husband, Lawrence. | :09:04. | :09:06.
Linda Harding knew Bernice and her family from services here. | :09:07. | :09:09.
Anybody who knew Bernice or knew the family will be very sad. | :09:10. | :09:15.
She just was a very genuine, quiet, sweet lady. | :09:16. | :09:23.
As a family, they came out as a family and supported one | :09:24. | :09:32.
People I have spoken to here today have told me they hadn't seen Bernie | :09:33. | :09:46.
or her husband in the village for some time. | :09:47. | :09:48.
Police say they are not looking for anyone else in connection | :09:49. | :09:50.
with the enquiry but tonight there are still more | :09:51. | :09:53.
questions than answers as to what actually happened. | :09:54. | :09:55.
When will police say more about what happened here? | :09:56. | :10:08.
We expect formal identification of the bodies found here to take place | :10:09. | :10:14.
later this week and then we should now a few more details about exactly | :10:15. | :10:20.
what happened. Luckily, it is believed the bodies are that of | :10:21. | :10:23.
Bernice Williams and her husband Laurence. All day, there has been a | :10:24. | :10:29.
lot of police activity and that is continuing tonight as the police | :10:30. | :10:36.
investigation continues. Thank you. More than 30 firefighters are | :10:37. | :10:39.
dealing with a large woodland fire in Lincolnshire. | :10:40. | :10:42.
They've been called to Laughton Woods. | :10:43. | :10:46.
It is not thought anyone has been hurt. | :10:47. | :10:48.
Nearly 100 council workers in East Yorkshire and Lincolnshire | :10:49. | :10:51.
The Taxpayers Alliance Town Hall Rich List shows 95 senior workers | :10:52. | :10:56.
in our area are earning six figures a year. | :10:57. | :10:58.
Lincolnshire County Council has the most high paid workers. | :10:59. | :11:00.
However overall councils are employing fewer people on a six | :11:01. | :11:02.
Pollution along a major road in Hull is higher than the safe limit | :11:03. | :11:12.
Figures show the concentration of pollutants on Castle Street has | :11:13. | :11:16.
increased by more than 20% in the past year. | :11:17. | :11:23.
Police in Lincolnshire are warning people who try to lie to get | :11:24. | :11:25.
out of speeding tickets that they will be caught out. | :11:26. | :11:28.
It comes after a man was been jailed for six months for perverting | :11:29. | :11:31.
Four years ago, Rashall-ul-Hassan could have gone on a speed awareness | :11:32. | :11:35.
course if he'd admitted going at 38 miles per hour in a 30 | :11:36. | :11:38.
Instead he tried to deceive the police and has ended up in jail. | :11:39. | :11:42.
It is not the most dramatic footage of a speeding offence but then this | :11:43. | :12:00.
wasn't one of the most serious speeding offences. It is street let, | :12:01. | :12:09.
it is a 30 mph limit and any residential area. At eight miles per | :12:10. | :12:12.
hour over the limit, the driver Rashall-ul-Hassan could have taken a | :12:13. | :12:19.
speed awareness course or a ?100 fine, but four years later after | :12:20. | :12:22.
trying to deny it was him at the wheel he has been jailed for six | :12:23. | :12:27.
months. That was a simple traffic violation. If he paid the fine I had | :12:28. | :12:35.
taken the course this would have gone away, but unfortunately he went | :12:36. | :12:39.
down this route and because of that he has been imprisoned and for the | :12:40. | :12:45.
rest of his life he will have a criminal record. It was just January | :12:46. | :12:54.
23rd 2013 when he broke the speed limit on this road. After giving a | :12:55. | :13:00.
false name and trying to deny it was him at the wheel, he was found | :13:01. | :13:04.
guilty and fined but then he complained to the central ticket | :13:05. | :13:07.
office and it was then investigation that found he lied all along. He | :13:08. | :13:12.
faced a much more serious charge of perverting the course of justice. | :13:13. | :13:18.
And that carries a much greater penalty as former Cabinet member | :13:19. | :13:23.
Chris Hughton found in 2013 when he was jailed for the same offence | :13:24. | :13:26.
after his wife at the time claimed she was driving to accept penalty | :13:27. | :13:33.
points so he could avoid a band. Today, a strong message in | :13:34. | :13:37.
Lincolnshire. If you have committed an offence is the easiest option is | :13:38. | :13:41.
to just admit it and get it behind you and move on with your life and | :13:42. | :13:48.
don't do something, that means you have two then tell moralise and | :13:49. | :13:58.
could end up in prison. Thank you for watching as always, still ahead. | :13:59. | :13:59.
The Lincolnshire driver who's going to take part in one | :14:00. | :14:01.
of the most famous races in the world. | :14:02. | :14:03.
The spectacular performance on a floating stage that's one | :14:04. | :14:05.
of the City of Culture's most ambitious shows. | :14:06. | :14:26.
Tonight's photograph is taken by Simon Wright. Thank you for that, | :14:27. | :14:34.
Simon, who will have a another photo tomorrow. Owain is here, and he is | :14:35. | :14:48.
wearing aquamarine, I am informed. I also have a Yuletide, what do you | :14:49. | :14:50.
think of that? I am going to tease you with some | :14:51. | :15:14.
good weather news, and we will see some more of that sunshine tomorrow. | :15:15. | :15:21.
Tonight, not a bad end to the day. A couple of spots of rain, a few | :15:22. | :15:26.
showers, blows of around eight or nine Celsius, and it will be breezy | :15:27. | :15:32.
if not blustery at times. There will be wind over the next couple of | :15:33. | :15:35.
days. Here are the times of high water tomorrow. Tomorrow is not too | :15:36. | :15:49.
bad, some rain, particularly further in land, but then it will brighten | :15:50. | :15:56.
up, we will see a little bit more in terms of sunshine or at least break | :15:57. | :16:00.
spells. There is room for improvement and tomorrow's whether | :16:01. | :16:06.
story. From the Humber, looking at around 13 Celsius, and a similar | :16:07. | :16:14.
story in land, but again those winds are a feature. Where has that | :16:15. | :16:18.
weather of last weekend gone, I hear you cry? It is not going to make a | :16:19. | :16:25.
visit this Easter weekend. A bridge of high pressure to the south, a | :16:26. | :16:31.
couple of weather fronts coming our way, but generally speaking, thanks | :16:32. | :16:36.
to that area of high pressure, the weather story is mostly dry and | :16:37. | :16:40.
settled. Watch out for the winds, but I think we will see some | :16:41. | :16:45.
sunshine, you know. You frightened as a little bit when you run | :16:46. | :16:50.
forwards to show as your tie, but you have teased many in the past, I | :16:51. | :16:52.
am sure. Thank you very much. A care home near Goole | :16:53. | :16:57.
in East Yorkshire is to close, 13 people live at Hook Hall, | :16:58. | :17:00.
which is the fourth care home in the East Riding to close | :17:01. | :17:03.
in the past 12 months. It comes as it is revealed more | :17:04. | :17:06.
than 90 social care workers quit their jobs every day | :17:07. | :17:09.
in Yorkshire and the Humber. Kate Sweeting is in | :17:10. | :17:15.
Hook Hall this evening. As you can see, this is a beautiful | :17:16. | :17:30.
building, and the loss of this care home has been described as a major | :17:31. | :17:32.
loss to this area. It has a good reputation locally, | :17:33. | :17:42.
and the 13 residents currently living here will have two move | :17:43. | :17:46.
elsewhere, which will have an impact on them. The owners have told me | :17:47. | :17:50.
today, that they cannot afford to continue. They cannot recruit enough | :17:51. | :17:56.
staff who can be flexible for the amount of money they can pay them, | :17:57. | :18:01.
and they do not get enough funding from the local authority to fund the | :18:02. | :18:06.
complex needs that there are residents have. One local councillor | :18:07. | :18:11.
said this. They are very undervalued, the work pays low, and | :18:12. | :18:23.
they can have far better terms of employment for the same money in | :18:24. | :18:27.
supermarkets. Many people had a lot of choice about when and where they | :18:28. | :18:34.
work. Town-mac has an 82% employment rate, and many people choose to work | :18:35. | :18:49.
in more flexible jobs. Goole has an 82% rate. This recruitment problem | :18:50. | :18:53.
is a national one, having an impact on local businesses like this one. | :18:54. | :19:05.
How to refund this industry, and how do we keep hold of the staff? We | :19:06. | :19:11.
will have more on this story, I will be talking to a cure home-worker | :19:12. | :19:15.
tonight at 10:30pm. Thanks to everyone who got in touch | :19:16. | :19:19.
about car parking after we told you East Lindsey Council | :19:20. | :19:22.
was to scrap free Not surprisingly, a big response on | :19:23. | :19:24.
that story. Thank you for those. Puffins are continuing to thrive | :19:25. | :20:03.
on the East Yorkshire coast despite a huge decline of the birds | :20:04. | :20:06.
across the world. It's believed a shortage | :20:07. | :20:08.
of food has affected birds in Iceland and Norway, | :20:09. | :20:10.
but a recent survey of the birds at Bempton Cliffs suggests numbers | :20:11. | :20:13.
here have remained stable What we are seeing right now | :20:14. | :20:15.
is that there seems to be enough sand eels to support the population | :20:16. | :20:18.
that we have here, but we are finding that | :20:19. | :20:20.
in other places around the world, that is simply not happening any | :20:21. | :20:23.
more because of climate change, | :20:24. | :20:25.
so the warmer waters mean That has not happened in | :20:26. | :20:27.
the North Sea yet, but that does not mean it could not | :20:28. | :20:31.
happen in the future. A Lincolnshire racing driver | :20:32. | :20:39.
will take part in one of the biggest Jack Harvey, who's from Bassingham | :20:40. | :20:42.
near Lincoln, will race in the world It is a long way from the karting | :20:43. | :20:46.
tracks of Lincolnshire, which he started racing | :20:47. | :20:51.
on as a schoolboy. We have been following | :20:52. | :20:53.
the career of Jack Harvey ever since he became | :20:54. | :20:59.
the youngest junior British karting | :21:00. | :21:02.
champion in 2006, at just 13 years I am about an inch away from sitting | :21:03. | :21:04.
on the floor, and when you are going down the straight, | :21:05. | :21:12.
everything passes you so fast. At 14, he won the European Italian | :21:13. | :21:15.
Masters Karting Championships and At 16, he moved up | :21:16. | :21:18.
to junior motor racing with Formula BMW, and by 17, | :21:19. | :21:24.
was leading in the championships. At 19, he became the | :21:25. | :21:30.
new British Formula 3 champion, a title previously | :21:31. | :21:33.
held by Nelson Piquet, and at 21, landed | :21:34. | :21:48.
place in the United States Indie Lights series, something else | :21:49. | :21:50.
But now, just 11 years on from all of this, he has | :21:51. | :21:54.
the chance to drive in the prestigious | :21:55. | :21:56.
Indy 500 at the Indianapolis Speedway. | :21:57. | :21:58.
It is the biggest race in America, with average speeds of over 230 mph. | :21:59. | :22:11.
To be in the Indy 500 is not an opportunity that a lot of people | :22:12. | :22:16.
have, and node to go and be a part of it, I do not have the words to | :22:17. | :22:26.
explain. -- now I am a part of it, I cannot explain what that means. At | :22:27. | :22:31.
his home, walls of medals are a reminder of what it has taken to get | :22:32. | :22:39.
this far. From the early days of karting, he has been so focused and | :22:40. | :22:43.
so dedicated, and he knows that every step he takes, he goes faster | :22:44. | :22:47.
and faster, and he smiles a bit, because I know he is thinking, just | :22:48. | :22:54.
support me, Mum. I have to be so dedicated, and that is what mums do. | :22:55. | :23:04.
To go there and see him doing it, I am proud. A bucket list moment. Yes. | :23:05. | :23:10.
The race will take place at the beginning of May. | :23:11. | :23:14.
It's about what happens when the end of the world approaches, and is one | :23:15. | :23:17.
of City of Culture's most ambitious events. | :23:18. | :23:19.
Flood is a year-long show played out on TV, | :23:20. | :23:21.
And tonight is the first of those live events, | :23:22. | :23:27.
in the middle of an old dock on a housing development in Hull. | :23:28. | :23:31.
Here's our culture correspondent, Anne-Marie Tasker. | :23:32. | :23:36.
I can't put one step in front of the next. | :23:37. | :23:40.
It's a story set in the future - asking how we'd cope if Europe | :23:41. | :23:43.
flooded and migrants washed up on the shores of a floating city. | :23:44. | :23:47.
And this week, almost 3,000 people will watch | :23:48. | :23:50.
from the sides of Victoria Dock listening through | :23:51. | :23:53.
headphones to the cast, out on the floating stage. | :23:54. | :23:57.
We knew that it would be much easier to tell a flood | :23:58. | :24:00.
story on water than it would to tell it on land. | :24:01. | :24:03.
It does come with a series of technical | :24:04. | :24:04.
challenges, but each of them allows an opportunity to create something | :24:05. | :24:08.
for the audience that they would not have seen elsewhere, so it is | :24:09. | :24:11.
The Leeds-based theatre company, SlungLow, spent more than two | :24:12. | :24:16.
weeks building the set, and then another fortnight | :24:17. | :24:19.
rehearsing what is the second part of a year-long project. | :24:20. | :24:26.
Part one saw a film released online, and toured around | :24:27. | :24:28.
I think it sounds like a radio play, and it looks a bit like an | :24:29. | :24:35.
Eventually, in the night-time, there will be 57 | :24:36. | :24:38.
It feels like there is a community talking and thinking | :24:39. | :24:46.
In that sense, it is really ancient Greek, so yes, it is | :24:47. | :24:50.
definitely a hybrid, and it is all on water, | :24:51. | :24:52.
Putting on an event like this, I have never seen anything | :24:53. | :24:56.
like it, and the open air, on the water. | :24:57. | :24:58.
I thought it was brilliant, the way it was all put together. | :24:59. | :25:01.
It was very interesting, very thought-provoking. | :25:02. | :25:03.
I do not think I breathed the entire way through. | :25:04. | :25:06.
That is one of the best things I have seen in a long time. | :25:07. | :25:10.
These live performances run until Saturday. | :25:11. | :25:11.
The next part of this epic piece of theatre will be shown on | :25:12. | :25:14.
BBC Two in the summer, before part four comes back | :25:15. | :25:17.
Good luck to them. I have seen them working hard over the last few weeks | :25:18. | :25:37.
in the cold. We will hear more about it on Friday, an interview with | :25:38. | :25:48.
Maureen Lipman, and Britain's City of Culture. | :25:49. | :25:57.
Let's get a recap of the national and regional headlines. | :25:58. | :26:01.
America's Secretary of State, Rex Tillerson, arrives in Moscow | :26:02. | :26:03.
to try and persuade Russia to end its support | :26:04. | :26:06.
A trust responsible for hospitals in Lincolnshire is put | :26:07. | :26:11.
back in special measures after patient safety is put at risk. | :26:12. | :26:15.
Tomorrow's weather - a cloudy and breezy morning | :26:16. | :26:17.
It will brighten up later with some sunny spells. | :26:18. | :26:20.
The maximum temperature will be 13 Celsius. | :26:21. | :26:32.
Response on the hospitals in Lincolnshire. Alex says, based on my | :26:33. | :26:39.
experiences over the last week with the birth of my sun, I cannot thank | :26:40. | :26:52.
the maternity department enough. Val says that I get excellent treatment | :26:53. | :26:58.
for my leukaemia. Ian says that if I had been given special measures by | :26:59. | :27:03.
my peers, I should not be paid for the jobs that I am doing. Why are | :27:04. | :27:08.
the bosses of these public sector... Sector organisations not being | :27:09. | :27:10.
sacked? Join me later at 10:30pm. There have never been | :27:11. | :27:34.
so many people in work - that's what the Government | :27:35. | :27:37.
keeps telling us. But what's the reality of this | :27:38. | :27:39.
Tory jobs bonanza? Well, if you're one of the millions | :27:40. | :27:43.
of people working on a contract without fixed hours | :27:44. | :27:48.
or days, then it's not so good. Nor is it so great if you're one of | :27:49. | :27:53.
the millions | :27:54. | :27:57. |
