In December 2012, Poppi Worthington collapsed at her home | :01:44. | :01:45. | |
She was taken to the local hospital but pronounced dead | :01:46. | :01:48. | |
there and the police began an investigation. | :01:49. | :01:50. | |
Yet more than four years on no one is any nearer to knowing why | :01:51. | :01:53. | |
The investigators found evidence that then an unstructured | :01:54. | :02:01. | |
investigation that was not fit the Earth. The evidence clues suggested | :02:02. | :02:04. | |
that basic investigative principles were not followed. | :02:05. | :02:10. | |
The IPCC report details a series of errors and omissions | :02:11. | :02:13. | |
The house was not sealed off and potentially vital | :02:14. | :02:16. | |
Poppy's aunt was allowed to throw away what could be | :02:17. | :02:23. | |
A concern that she may have been seriously sexually abused | :02:24. | :02:26. | |
but the IPCC says detectives continued | :02:27. | :02:29. | |
And witness accounts weren't taken until eight months after her death, | :02:30. | :02:33. | |
At that point her father Paul Worthington was questioned | :02:34. | :02:37. | |
He denied it, and both parents were released without charge. | :02:38. | :02:43. | |
An inquest into her death lasted seven minutes and recorded the cause | :02:44. | :02:47. | |
But at the beginning of 2016 a family court judge ruled that | :02:48. | :02:57. | |
Paul Worthington probably did sexually abuse his daughter. | :02:58. | :03:00. | |
Something he has always strongly denied. | :03:01. | :03:03. | |
The Crown Prosecution looked at the case again - | :03:04. | :03:06. | |
but ruled that the police didn't have enough evidence | :03:07. | :03:08. | |
By the time the Trail was picked up, it had gone cold. So probably, there | :03:09. | :03:24. | |
will be never be justice for Poppi Worthington and that is a terrible | :03:25. | :03:29. | |
thing. For that baby girl, and of course the whole community. | :03:30. | :03:32. | |
Two officers were singled out for criticism. | :03:33. | :03:33. | |
Detective Superintendent Forrester retired during the investigation | :03:34. | :03:35. | |
and Detective Inspector Sadler was demoted and then also retired. | :03:36. | :03:40. | |
Today, calls for a change in the rules by the local MP | :03:41. | :03:47. | |
DS Forrester today released a statement | :03:48. | :03:49. | |
saying he was truly sorry that they were not able to establish | :03:50. | :03:52. | |
the cause of Poppy's death and for the mistakes made | :03:53. | :03:54. | |
but that the IPCC didn't live in the real world, | :03:55. | :03:59. | |
The view that this investigation was not fit the purposes by the family, | :04:00. | :04:04. | |
High Court judge and Chief Constable, who has accepted that the | :04:05. | :04:13. | |
force that Poppi down. -- let Poppi down. | :04:14. | :04:14. | |
A new inquest will be held into Poppi's death in May. | :04:15. | :04:17. | |
Jerry Graham took over the post of Cumbria's Chief Constable | :04:18. | :04:19. | |
in 2014 almost two years after Poppi's death. | :04:20. | :04:21. | |
I spoke to him earlier and asked for his response to the IPCC's | :04:22. | :04:25. | |
I am very clear that the initial investigation into Poppi | :04:26. | :04:29. | |
Worthington's death fell well short of what Poppi's family | :04:30. | :04:33. | |
could have expected and should have | :04:34. | :04:34. | |
And I would like to give a heartfelt apology to Poppi's family | :04:35. | :04:41. | |
for the inadequacies in Cumbria Constabulary's initial | :04:42. | :04:43. | |
The IPCC report does mention two senior officers by name. | :04:44. | :04:52. | |
And says they have cases to answer for gross misconduct. | :04:53. | :04:54. | |
Neither did face those charges and both have now | :04:55. | :04:58. | |
retired, presumably on a full police pension. | :04:59. | :05:01. | |
Both officers would have faced gross incompetence under misconduct | :05:02. | :05:09. | |
The most senior officer officer retired from the | :05:10. | :05:18. | |
police force, as was his right under regulation. | :05:19. | :05:25. | |
But should he have been allowed to retire while | :05:26. | :05:27. | |
Well, the government is looking at legislation to change. | :05:28. | :05:31. | |
Because the public will have the review, is that | :05:32. | :05:33. | |
And I have some sympathy with that position, because I think that | :05:34. | :05:37. | |
the family of Poppi Worthington and the public need | :05:38. | :05:39. | |
But under existing regulations, that offer was able to retire. | :05:40. | :05:43. | |
The second officer was a detective inspector, and did face gross | :05:44. | :05:46. | |
incompetence and indeed, got a sanction. | :05:47. | :05:48. | |
These were not bad people who did a bad job, | :05:49. | :05:56. | |
who made serious mistakes in this particular case. | :05:57. | :06:07. | |
Can anyone be really confident that nothing like | :06:08. | :06:09. | |
This incident happened in December 2012. | :06:10. | :06:12. | |
It first came to my notice in the middle | :06:13. | :06:14. | |
hard since 2014, to this day, to make sure | :06:15. | :06:18. | |
