A doctor admits he did not read medical notes properly,

:00:08.:00:15.

If they had just looked at the notes and treated my sister

:00:16.:00:20.

as a normal human being, she would still be here.

:00:21.:00:23.

Sentencing begins for two violent robbers who poured boiling water

:00:24.:00:26.

over a retired couple in a terrifying two-hour

:00:27.:00:28.

how criminals are using Facebook to sell dangerous illegal cigarettes

:00:29.:00:38.

The Sussex twin whose brain tumour was spotted by chance,

:00:39.:00:49.

now raising money with her sister for cancer research.

:00:50.:00:53.

I didn't get to see my sister a lot and I love her.

:00:54.:00:58.

And Miami's nice for Johanna Konta,

:00:59.:00:59.

the first British woman ever to make the final of the Miami Open.

:01:00.:01:18.

A doctor has admitted that the life of a Kent mother-of-three might

:01:19.:01:22.

well have been saved, if he had read her medical notes properly.

:01:23.:01:25.

The admission sparked emotional scenes at the inquest

:01:26.:01:27.

into the death of Sian Hollands, who was 25 when she died

:01:28.:01:30.

at the Darent Valley Hospital in Dartford.

:01:31.:01:31.

She suffered a pulmonary embolism just hours after doctors had decided

:01:32.:01:34.

This afternoon, her family ran from the inquest

:01:35.:01:42.

in Gravesend in tears, after hearing evidence

:01:43.:01:44.

from Dr Kamran Khan, who had previously claimed he had not

:01:45.:01:46.

had access to her notes. Peter Whittlesea has the details.

:01:47.:01:59.

For the family, the inquest is all about uncovering the truth. Today,

:02:00.:02:07.

what was described in court a significant new evidence highlighted

:02:08.:02:12.

by doctors failed to diagnose the embolism which killed her. If they

:02:13.:02:18.

had just looked at the notes and treated her as a normal human being,

:02:19.:02:23.

my sister would still be here. That is what is so hard. When you hear

:02:24.:02:32.

that in court, you had to leave the court? Yes, I had to leave. It was

:02:33.:02:36.

too difficult for me. Do you believe that you have no go the truth? Yes,

:02:37.:02:45.

I am relieved. For the entire family. Doctor Sidney had never seen

:02:46.:02:50.

notes saying she was having difficulty breathing and the chest

:02:51.:02:55.

pains which led to the wrong diagnosis. Today, the notes were

:02:56.:03:01.

found in the notes handwritten by Dr Kamran Khan which led to a dramatic

:03:02.:03:09.

courtroom exchange. The family barrister said it means you could

:03:10.:03:14.

not have read what was stealing you in the face? The doctor replied yes.

:03:15.:03:21.

The doctor said, you simply did not read the notes. The doctor

:03:22.:03:25.

responded. I should have read the notes.

:03:26.:03:37.

In court, the hospital trust expressed deepest sympathy to the

:03:38.:03:48.

family. The family in Tom said these were empty words. If they had simply

:03:49.:03:52.

read the notes, she would be standing here and alive and well.

:03:53.:03:57.

The inquest has now been adjourned for the coroner to consider his

:03:58.:03:58.

verdict. And he is in Gravesend, where

:03:59.:04:00.

the inquest has been taking place. Peter, there have been

:04:01.:04:04.

five days of evidence, so what is next for the family

:04:05.:04:06.

of Sian Hollands? The coroner said after five days of

:04:07.:04:12.

a lot of medical evidence to consider, he will be recording his

:04:13.:04:18.

verdict on April the tense. He apologised to the family for another

:04:19.:04:23.

delay. But at these dramatic scenes with a family here that the Doctor

:04:24.:04:29.

failed to turn over a page, the mother said after 500 pages -- 500

:04:30.:04:36.

days after her death, they finally realised why the Hadfield to

:04:37.:04:42.

diagnose her condition. The general medical Council will now consider

:04:43.:04:43.

the doctors rule in her death. Sentencing has begun for two violent

:04:44.:04:49.

criminals who poured boiling water over a retired couple, in

:04:50.:04:52.

an horrific ordeal that lasted John and Janis Buswell

:04:53.:04:54.

were tied up and tortured in their home in Halstead,

:04:55.:04:57.

near Sevenoaks, last April. Both suffered life-changing

:04:58.:05:00.

injuries, as a result out by Daniel Wallace

:05:01.:05:01.

and Kacey Adams. Mrs Buswell spent

:05:02.:05:04.

months in intensive care. As Piers Hopkirk reports,

:05:05.:05:05.

they say it has left them physically and mentally scarred

:05:06.:05:08.

and feeling their lives Daniel Wallace and Kacey Adams.

:05:09.:05:29.

Violent criminals. The two hour attack on the pensioners were so

:05:30.:05:33.

shocking it received nationwide coverage. It is a day that victims

:05:34.:05:44.

have to live with out each day of their lives. These people forced to

:05:45.:05:49.

the address and the author of the brand wireless level of violence

:05:50.:05:55.

against them. That included having boiling water poured over them. It

:05:56.:06:03.

happened at the couple torn. It was an act of such an unprecedented

:06:04.:06:06.

violence with absolutely stunned the community. We were absolutely

:06:07.:06:14.

horrified. Especially cause, I suppose it could have happened to

:06:15.:06:21.

anyone here. My name? It is absolutely terrible for the dead.

:06:22.:06:29.

Especially to his wife. They should get badly punished. The court here

:06:30.:06:33.

that the two of them had tortured them to get the location of two

:06:34.:06:38.

safes. After a prolonged period of threats and violence, they got them,

:06:39.:06:43.

leaving the home with more than ?40,000 in cash. Janis Buswell And

:06:44.:06:53.

such severe injuries, she later fell into a coma. The victim statement

:06:54.:06:54.

read to court said. The two men will be sentenced on

:06:55.:07:20.

Monday. Piers Hopkirk reporting and he is

:07:21.:07:23.

live at Chelmsford Crown Court. The victims did not just suffer

:07:24.:07:34.

physically and mentally. It has had a profound psychological effect. We

:07:35.:07:46.

here this afternoon that Janice suffered from PTSD as a result of

:07:47.:07:53.

the attack. The couple say the also felt almost compelled to seal the

:07:54.:08:01.

hosts and positions because the field some who contaminated by both

:08:02.:08:09.

being back in the house and touching anything which the main meat have

:08:10.:08:13.

touched. The men will be sentenced on Monday.

:08:14.:08:16.

In a moment, we are speaking to the former Royal Marines who have

:08:17.:08:19.

kayaked round the Falklands to raise funds and awareness

:08:20.:08:21.

A BBC South East undercover investigation has revealed that

:08:22.:08:32.

criminal gangs are using Facebook to sell fake cigarettes

:08:33.:08:34.

which contain dangerously high levels of lead

:08:35.:08:36.

Trading Standards says it is a deeply-concerning

:08:37.:08:39.

which is endangering people's health and short-changing the economy.

:08:40.:08:42.

Glen Campbell has this exclusive report.

:08:43.:08:50.

Facebook recently launched a new concept in local trading.

:08:51.:08:52.

It is called Marketplace and makes buying and selling goods

:08:53.:08:55.

We've received a tip-off there are more questionable goods being sold

:08:56.:09:04.

Goods that would normally be seen being knocked out

:09:05.:09:07.

the back of a white van, cut-price tobacco and cigarettes

:09:08.:09:09.

Criminals are selling tobacco using fake profiles to avoid detection,

:09:10.:09:13.

which is why we have arranged to meet them face-to-face.

:09:14.:09:23.

The stint disgruntled customer bought 200. Therefore much more

:09:24.:09:27.

harsh than normal cigarettes. So, how much extra substances in

:09:28.:09:47.

the? The level of lead is about double. The amount of cadmium is

:09:48.:09:55.

about 30%. The likelihood is you're going to be taking in more of these

:09:56.:09:59.

contaminants. It will remain in your system. In your organs, in your

:10:00.:10:04.

brain. I just how easy is it to get your hands on this questionable

:10:05.:10:09.

tobacco. We went online and phone the seller we had been tipped off

:10:10.:10:10.

about. We have had a message on Facebook

:10:11.:10:22.

telling us to turn up at a leisure I try to find out

:10:23.:10:26.

why it was so cheap. And you keep the change

:10:27.:10:31.

and we are all happy. We sent the tobacco off to the

:10:32.:10:47.

original manufacturer. They said these were counterfeit. How much of

:10:48.:10:54.

a problem is this sale of counterfeit tobacco on social media

:10:55.:10:59.

become? In recent years, the amount of reports of illicit tobacco on

:11:00.:11:05.

social media has increased. It is about 20% no. It is also very

:11:06.:11:11.

difficult to find out who the borough. They using fake profiles to

:11:12.:11:18.

avoid detection. The tool does that they have reviews -- remove the

:11:19.:11:24.

advert selling the cigarettes because it contradicts the sales

:11:25.:11:31.

policy. But soon after, another felt appeared. -- advert.

:11:32.:11:39.

Juliette Parkin has been following the story and she is live

:11:40.:11:41.

at Kent Police Headquarters in Maidstone.

:11:42.:11:43.

Juliette, what have the police had to say about this tonight?

:11:44.:11:48.

They say they have made the arrests in connection with the sale of

:11:49.:11:53.

counterfeit tobacco in the region. They are also asking ferry man with

:11:54.:11:57.

information to come forward. Wireless or concealed? We do not

:11:58.:12:04.

think that anyone involved in a film was involved any wider criminal

:12:05.:12:08.

network, they are worried that it could attract criminal gains. They

:12:09.:12:13.

also worry that the profits from this could go to the likes of people

:12:14.:12:20.

trafficking or drug smuggling. This view is that what is behind what we

:12:21.:12:25.

have seen is a more concerted criminal enterprise.

:12:26.:12:27.

And you can see what happened when a multi-national tobacco

:12:28.:12:29.

company carried out an analysis of the fake cigarettes

:12:30.:12:31.

on Inside Out, here on BBC One, at 7.30 this evening.

:12:32.:12:34.

A parliamentary aide to a Kent MP has appeared

:12:35.:12:36.

at Westminster Magistrates Court, charged with two counts of rape.

:12:37.:12:38.

Samuel Armstrong, who was chief of staff for the South Thanet MP

:12:39.:12:41.

Craig Mackinlay, has been suspended since his arrest in October.

:12:42.:12:44.

He has been bailed to appear before Southwark Crown Court next month.

:12:45.:12:50.

The pilot and owner of a plane that ditched into the sea off the

:12:51.:12:53.

Sussex coast yesterday says the engine cut out

:12:54.:12:55.

Ian Perry swam to shore with his passenger, after the light

:12:56.:13:00.

aircraft came down into the water shortly after take-off

:13:01.:13:02.

from Shoreham Airport, where he runs a flying school.

:13:03.:13:05.

Insurance assessors have been examining the plane today.

:13:06.:13:06.

It is likely to be written off, due to water damage.

:13:07.:13:16.

Care home providers are pushing up prices for private clients,

:13:17.:13:19.

because they are not getting enough funding from local councils,

:13:20.:13:21.

according to an influential group of MPs.

:13:22.:13:23.

The Communities and Local Government Committee says it is "unacceptable"

:13:24.:13:25.

that people who pay their own fees at care homes are charged 43%

:13:26.:13:28.

more on average than council-funded residents

:13:29.:13:30.

Councils are short of money. The Arab trying to get the best place

:13:31.:13:49.

they can. They are running fairly small margins and try and make the

:13:50.:13:50.

difference that way. Our Political Editor Helen Catt

:13:51.:13:54.

is with me in the studio. Helen, MPs heard claims that

:13:55.:13:57.

Kent County Council told care homes they should seek to make a profit

:13:58.:14:00.

from privately-funded residents. Yes, among the many groups which

:14:01.:14:13.

Dave evidence of those who represent the care providers in Kent. They

:14:14.:14:18.

said county county council representatives and openly admitted

:14:19.:14:23.

that they should look to make profits from privately funded

:14:24.:14:27.

service users. It is because of us that they set the fees soar. Some of

:14:28.:14:34.

the industry see this should not happen is wrong. This should be

:14:35.:14:44.

charging the rate for social care. A local authority to eventually see go

:14:45.:14:47.

ahead and charge them all in order to give us a bit of a break, I think

:14:48.:14:54.

that is a Regis. The county council disputes this and says the comment

:14:55.:14:59.

does not reflect the view or policy. This theatre is not the role of the

:15:00.:15:05.

Council care detail providers how to set the fees. We have asked for any

:15:06.:15:09.

evidence to be provided which backs up that claim.

:15:10.:15:10.

A doctor has admitted that the life of a Kent mother-of-three might well

:15:11.:15:15.

have been saved, if he had simply taken the trouble

:15:16.:15:18.

Also in tonight's programme, Venus Williams defeated,

:15:19.:15:22.

now Caroline Wozniacki awaits, as Eastbourne's Johanna Konta

:15:23.:15:24.

reaches the final of the prestigious Miami Open.

:15:25.:15:39.

And after about dumpster, things in the afternoon. How are things

:15:40.:15:48.

looking for the weekend? The forecast coming up.

:15:49.:15:53.

A mother from Eastbourne is campaigning to raise awareness

:15:54.:15:55.

of the risk of brain tumours, after one of her nine-year-old

:15:56.:15:58.

twins was diagnosed, without having displayed

:15:59.:15:59.

Becki Charman's daughter Isabelle had been suffering from backache,

:16:00.:16:02.

but a doctor chose to scan her eye and realised a tumour

:16:03.:16:05.

16,000 people are diagnosed every year with the condition,

:16:06.:16:08.

which kills more patients under 40 than any other form of cancer.

:16:09.:16:11.

Brain tumours account for 3% of all new UK cancer cases,

:16:12.:16:14.

but attract just 1% of national cancer research funding.

:16:15.:16:16.

Today, one year on from her treatment,

:16:17.:16:18.

Isabelle and her twin sister have been fundraising

:16:19.:16:19.

Our Education Correspondent Bryony MacKenzie has

:16:20.:16:22.

Twins Matilda and Isabelle do most things together.

:16:23.:16:31.

So when Isabelle was diagnosed with a brain tumour, what

:16:32.:16:33.

The bad thing was I didn't get to see my sister a lot.

:16:34.:16:39.

I didn't really like having a lot of injections.

:16:40.:16:52.

She had an eight-hour operation to remove a brain tumour a year

:16:53.:16:55.

Her mum says it came out of the blue.

:16:56.:16:58.

She had no classic symptoms of what you think

:16:59.:17:08.

would be associated with a brain tumour.

:17:09.:17:10.

The twins and their school are wearing hats to raise money and

:17:11.:17:13.

awareness about brain tumours in children.

:17:14.:17:15.

Headaches, being sick all the time, and unusual things that

:17:16.:17:20.

Like being sick in the night every night.

:17:21.:17:28.

You wouldn't normally have a headache.

:17:29.:17:31.

But people mainly think that you would if you had a brain

:17:32.:17:35.

tumour, you would always get headaches, but you don't.

:17:36.:17:40.

With the support of her local MP, Becki is passing on a simple

:17:41.:17:43.

My own son had a very similar experience a few years ago. He had

:17:44.:18:04.

incredible treatment in London in his life was saved. It is vital we

:18:05.:18:09.

reason we are some vital funds to power research going forward.

:18:10.:18:13.

With the support of her local MP, Becki is passing on a

:18:14.:18:16.

Just say, could you look behind my daughter's eyes or

:18:17.:18:22.

Bryony Mackenzie, BBC South Today, Eastbourne.

:18:23.:18:35.

They braved freezing temperatures, gale force winds and even

:18:36.:18:37.

But former Royal Marines Mick Dawson and Steve Grenham, from Brighton,

:18:38.:18:44.

have made it back home after a gruelling eight-day kayaking

:18:45.:18:47.

trip around the coastline of the Falkland Islands.

:18:48.:18:49.

They have been raising thousands of pounds for charity

:18:50.:18:51.

and awareness of Post-Traumatic Stress Disorder among former

:18:52.:18:53.

servicemen and women - something that Steve has experienced

:18:54.:18:55.

In a moment we'll have a chat with Mick and Steve here in the studio.

:18:56.:19:06.

But first, here is a taste of their Falklands expedition.

:19:07.:20:00.

Mick Dawson and Steve Grenham, many thanks for coming in.

:20:01.:20:02.

What was the toughest moment and biggest challenge?

:20:03.:20:22.

I think we had one tough day on the second ballast one. It was the

:20:23.:20:31.

shortest route on paper, but it was rates and to hear twins. It was very

:20:32.:20:36.

difficult. But not one specific moment. You had some amazing

:20:37.:20:46.

experiences. We saw the dolphins. The company due into Port Stanley?

:20:47.:20:50.

Yes, it was amazing. Why do you feel Post-Traumatic

:20:51.:20:54.

Stress Disorder needs Even like for his 50th birthday last

:20:55.:21:15.

year. He explained to me that he had been looking for help with issues to

:21:16.:21:21.

do with PTSD. He said he could not get help and I thought, that we

:21:22.:21:25.

should then set up a project to help them get through it. I was meant to

:21:26.:21:31.

be a three-month project. Two months later, we are still going. Do you

:21:32.:21:40.

think raising awareness helps? I think it is a huge part. There were

:21:41.:21:50.

other groups which had been formed, but they did not appeal to me. This

:21:51.:22:03.

then. It was incredible to be back there. Some of the walking we did

:22:04.:22:08.

back in 1982 was incredible. What sort of support would you like to

:22:09.:22:14.

see? There is more now than there ever was. There was nothing in our

:22:15.:22:20.

day. What is the most effective sort of help? It can be different for

:22:21.:22:28.

every person? I read a bit up on the Internet. I read about it and just

:22:29.:22:38.

read like my life story. I would say, if it wouldn't save it in from

:22:39.:22:47.

the Falklands, the sugar down and have a look. It could put some

:22:48.:22:52.

ghosts to rest. And people can fall your experiences online? Yes, we are

:22:53.:22:59.

online and on Facebook. Thank you both very much for coming in.

:23:00.:23:04.

On to tennis now and Eastbourne's Johanna Konta has become the first

:23:05.:23:07.

British woman ever to make it through to the final

:23:08.:23:10.

of the Miami Open - one of the most prestigious events

:23:11.:23:12.

The British number one beat seven-time grand slam winner

:23:13.:23:16.

Venus Williams in straight sets and is now preparing to take

:23:17.:23:18.

on Caroline Wozniacki in tomorrow's final.

:23:19.:23:20.

Johanna Konta striding out for the showdown semifinal

:23:21.:23:27.

against the woman she has described as one of her heroes.

:23:28.:23:31.

Seven-time Grand Slam winner Venus Williams first won

:23:32.:23:33.

But the British number one was not here to be intimidated.

:23:34.:23:41.

Instead, she rallied her way to a third successive victory

:23:42.:23:44.

There was so little in it that I was very happy

:23:45.:23:49.

I wasn't able to serve it out the first time,

:23:50.:23:54.

but the second time around, I was able to serve.

:23:55.:23:57.

Yes, relief and excitement that I get to be part of the last

:23:58.:24:00.

She broke Williams twice in the first set,

:24:01.:24:07.

The second set was a different story, with the Sussex

:24:08.:24:11.

star having to break the American on three occasions

:24:12.:24:13.

setting up a showpiece meeting with former world number one

:24:14.:24:26.

I get to play against an incredibly tough opponent, someone who has been

:24:27.:24:30.

at the top of the game for so long now.

:24:31.:24:33.

So, overall, just to enjoy the challenges of what will come.

:24:34.:24:35.

If she lifts the trophy tomorrow, Johanna Konta will reach

:24:36.:24:38.

a career-high world number seven ranking.

:24:39.:24:39.

Good luck. You are looking very springlike. Is that a reflection of

:24:40.:24:57.

the weather over the weekend? Not quite as much sunshine as we

:24:58.:25:17.

have seen in recent days. Cloud The boost its weakest words and at the

:25:18.:25:22.

plating up as the afternoon progressed. The average temperature

:25:23.:25:32.

11-12 C. Very mild and I will continue. Saturday, we are expecting

:25:33.:25:39.

to see some scattered showers. Into Sunday, high pressure building and

:25:40.:25:48.

it will be a lovely lot of sunshine. Temperatures 15-16 C. Tonight,

:25:49.:25:56.

relatively light wind. A bit more cloud cover. Temperatures dropping

:25:57.:26:03.

overnight into single figures. Still mild very, but nothing like the

:26:04.:26:09.

overnight temperatures we have seen recently. A cool start to the day

:26:10.:26:18.
