He had complex mental health problems, but a new report -

seen exclusively by BBC Wales - says he would still be alive

had he received the care he needed.

The charity, Hafal, is now calling on the Welsh Government

to do more to ensure health boards comply

Their little sunshine - that's what Chris Wood's family

called him when he was a child.

But life became very difficult when he developed

a borderline personality disorder.

It meant he couldn't cope with intense emotions

and would often self harm.

He tried to kill himself 32 times in his short life.

In January 2013, he went missing and his parents, Debbie and Robin,

found him in a lane.

He had hanged himself.

We had taken him to A so many times, but always

brought him back with us, that there was a big part of me that

really couldn't quite believe it. It was just utterly horrendous.

For my lovely son, we'd fought so hard to keep safe,

to die in the street like that...

To try and explain that to my husband's 90-year-old parents

was just unbelievable.

Um, it was horrendous, it really was.

A new report has found that Chris's death would've been prevented had

he received a reasonable standard of care, specifically a care

0:02:020:02:04

coordinator to oversee a consistent approach to his complex problems.

Debbie said that reading this report was devastating.

It found that there was no evidence that Chris's risk to himself

was correctly assessed the year before he died.

It also says that his care plan was inadequate and that there

was grave concern about the lack of the involvement of the family.

Now there are calls for the Welsh Government to do more

to ensure that services comply with mental health law.

We hear too much, "We've learned lessons from these sorts of cases."

Enough is enough and if people don't want to deliver to the mental health

measure, they should give up delivering

the mental health services

themselves and give it to a health board thats capable of doing that.

The Welsh Government says that mental health will be

a priority for them.

They say that they expect services to comply with national guidelines

and that it's introduced a measure to ensure all those in secondary

Cardiff and Vale University Health Board has apologised to Chris's

family and said they've taken steps to address the issues raised

in the report.

It also says it's been developing services for people

with problems like Chris'.

It's been a three-year fight for Debbie and her family.

They're speaking out now, because they want changes for others

like Chris, who have complex mental health needs.

Personally, I wouldn't do it, because I'd use the money for,

I think, the pressing thing - the most pressing thing of all -

which is, independent of class sizes, we haven't developed

the capacities of our teachers to be world-class great teachers.

That's what I would spend my money, but then, I'm not in politics.

You speak to any teacher, they'll tell you that,

if they have a smaller class size, they can offer better

services to those pupils, they can identify strengths

or weaknesses easier. They have better time for planning.

But I'd have some concerns about the debate being do we reduce

class sizes or do we improve teachers' professional development?

It shouldn't be an either or, it should be how do we ensure

a situation where we do both of those things?

And staying with schools - a consultation on the future

of four Secondary Schools in Powys has closed.

The council has plans to close Gwernyfed and Brecon schools,

and create a single school in their place, doing
