-Midwives...

-Hello!

0:00:020:00:04

BABY CRIES

0:00:040:00:06

They have to be there when they're needed.

0:00:060:00:08

My baby!

0:00:080:00:09

Being mother to the mothers...

0:00:090:00:11

You're waters haven't gone, have they?

0:00:110:00:14

..support to the fathers....

0:00:140:00:16

Yeah, yeah, fine.

0:00:160:00:17

..and trusted colleagues to each other.

0:00:170:00:19

All dressed, ready to go.

0:00:190:00:21

But, in this part of the world, their journey is never easy.

0:00:210:00:25

Oh, 'eck!

0:00:250:00:26

It's full of great ups and difficult downs...

0:00:260:00:28

It's through gates, over fences...

0:00:280:00:31

..of twists and turns...

0:00:310:00:33

Most bizarre places... Up a tree house, actually.

0:00:330:00:36

..of never quite knowing what's round the corner.

0:00:360:00:38

A total bag of emotions.

0:00:380:00:41

Welcome to the world of safe hands...

0:00:410:00:43

You've got a lovely belly.

0:00:430:00:44

My office is my car, you know.

0:00:440:00:46

..all-seeing eyes...

0:00:460:00:48

Absolutely perfect.

0:00:480:00:50

..and big, big smiles.

0:00:500:00:51

Very au naturel.

0:00:510:00:53

I think a lot of the women are quite strong characters.

0:00:530:00:56

Welcome to the real-life drama of The Country Midwives.

0:00:560:00:59

Carmarthenshire is one of Wales' most rural counties,

0:01:050:01:08

whose expectant mums are looked after by the Hywel Dda

0:01:080:01:11

University Health Board's midwives.

0:01:110:01:14

To the north - in the Teifi Valley - is the village of Glangwili

0:01:140:01:17

and its neighbouring farming communities.

0:01:170:01:20

It's very beautiful in summer.

0:01:200:01:22

Winter is something different, as community midwife Anwen Evans

0:01:220:01:27

knows only too well.

0:01:270:01:28

It's fun and games.

0:01:300:01:32

Because I live on a farm, Friday morning I got up at quarter past six

0:01:320:01:36

to take my works car to the bottom of the lane so I wouldn't be stuck.

0:01:360:01:40

I'm on call tonight, so if I get called out,

0:01:400:01:43

I'll have to get Sean out of bed to take me down to my car -

0:01:430:01:47

in the 4x4. He'll be impressed.

0:01:470:01:50

Anwen tackles over a thousand miles a month of Carmarthenshire's

0:01:520:01:55

maze of country roads and farm tracks.

0:01:550:01:58

She's calling on farm wife Bethan, who's 30 weeks pregnant.

0:01:580:02:02

It's her first time. And it's twins!

0:02:020:02:05

For midwives, a first-time pregnancy means extra care.

0:02:160:02:20

A first time and twins means extra, extra care.

0:02:200:02:23

We do try to see Bethan once a week.

0:02:250:02:29

Whether it be us in the community or the ante-natal clinic.

0:02:290:02:32

We check her blood pressure to make sure everything's OK.

0:02:320:02:35

We check her urine to make sure there's no infections.

0:02:350:02:39

We check the positions of the baby

0:02:390:02:41

and we listen to the babies' heartbeats - both of them.

0:02:410:02:44

WHOOSHING

0:02:440:02:47

Perfect.

0:02:470:02:48

Both happy.

0:02:500:02:52

Husband Dylan is in the middle of lambing -

0:02:540:02:57

a time for great care, as sheep and lambs carry

0:02:570:03:00

a parasitic disease, dangerous for both mother and child.

0:03:000:03:04

Be careful, don't go into the lambing shed because of the disease, toxoplasmosis,

0:03:050:03:10

which could have an effect on your pregnancy, and Dylan needs to

0:03:100:03:14

wash his own clothes and stuff, when he comes in from the farm.

0:03:140:03:19

Anwen learnt the dos and don'ts of being a farm wife

0:03:190:03:23

and mum-to-be from her own, personal, experience.

0:03:230:03:26

Because I live on a farm, I couldn't go out to the lambing shed,

0:03:260:03:30

and then in my clothes go and see a woman.

0:03:300:03:32

I would have to be careful to get changed.

0:03:320:03:35

Or just avoid the lambing shed. It's a good excuse, really.

0:03:350:03:39

In her garden, near Carmarthen, mother and midwife Sian Maynard

0:03:430:03:47

is not only concerned with feeding the birds.

0:03:470:03:50

As team leader to Anwen and colleagues, she has to ensure

0:03:500:03:53

that all expectant mums are attended to, whatever the weather.

0:03:530:03:56

Hello, could you put me through to the community office, please?

0:03:580:04:01

Is it busy on the ward now?

0:04:010:04:03

You got into work OK, then?

0:04:030:04:05

Are you going to keep me till later this afternoon to come in?

0:04:050:04:08

I've written up what I think should be in the report.

0:04:080:04:11

OK, see you soon. Ta-ra.

0:04:110:04:13

I'll recheck the annual leave today.

0:04:130:04:15

Unfortunately, I only live two miles from the hospital so there's a little

0:04:150:04:21

sort of handful of us that live within walking distance, so when the weather is like this,

0:04:210:04:26

we are the ones that are on stand-by.

0:04:260:04:29

Because our area is so rural, there are pockets that are

0:04:290:04:32

going to be really bad. If anybody does have problems,

0:04:320:04:36

we get the ambulance out to them.

0:04:360:04:40

We just come across that at the time.

0:04:400:04:44

Last resort, helicopter.

0:04:440:04:46

A comfy hop in a handy helicopter

0:04:480:04:50

would be very welcome for Sian herself this morning.

0:04:500:04:53

The reality, unfortunately, is a bit different.

0:04:530:04:57

Blinking 'eck, it's cold.

0:04:570:04:58

I think my ears are going to drop off at any moment.

0:04:580:05:03

Hopefully, she needn't go further today than her Glangwili

0:05:040:05:07

hospital base at nearby Carmarthen.

0:05:070:05:10

Carmarthenshire's eastern edge -

0:05:130:05:15

particularly around Llandeilo - is also very rural.

0:05:150:05:19

It's served by enough B and C roads to confuse the best sat-nav.

0:05:190:05:23

Area midwife Nan Duncan isn't fazed by the weather.

0:05:230:05:27

But what of the hours?

0:05:270:05:28

17 hours yesterday, so I was in bed about 2:30ish and eventually

0:05:280:05:34

went to sleep and woke up at eight o'clock this morning.

0:05:340:05:38

Got up and at it, so I'm here.

0:05:380:05:40

This is a knitted breast. We show people how to express breast milk.

0:05:410:05:47

So would you like a demonstration?

0:05:470:05:49

You pull back and you squeeze -

0:05:530:05:55

that's how you get some milk.

0:05:550:05:57

Excellent, actually, it works.

0:05:570:05:59

Hiya, how you doing?

0:06:010:06:04

Helping at the time of birth is only a small part of a midwife's work.

0:06:040:06:08

Their responsibility starts at around the tenth week of pregnancy

0:06:080:06:12

and will continue until at least ten days after birth.

0:06:120:06:15

-Famous question - have you pooed?

-We saved you a nappy.

0:06:150:06:19

SHE LAUGHS

0:06:190:06:21

I like it, I like it.

0:06:210:06:22

-Can I have a look at it?

-Yeah.

0:06:220:06:24

Aw. Isn't that the best?!

0:06:240:06:27

Hang on, hang on. You've got to see this.

0:06:270:06:31

This is best breastfeeding poo.

0:06:340:06:37

I'm so proud. Yeah. So proud.

0:06:390:06:42

It takes time to get breastfeeding going,

0:06:420:06:45

but when you get lots of poo-y and wee-y nappies, it's just champion.

0:06:450:06:50

When they are born they've got brown fat which keeps them going

0:06:500:06:54

while the breastfeeding gets initiated.

0:06:540:06:57

So there's a natural fall-off of weight.

0:06:570:07:01

Then as the breastfeeding gets established, there is

0:07:010:07:05

a gain in weight, so Helen has worked really hard

0:07:050:07:08

because it is tiring, to say the least.

0:07:080:07:11

Becoming a parent is awful, it's hard work.

0:07:110:07:14

So I'm really chuffed -

0:07:140:07:16

-are you chuffed?

-Yeah, getting there. Just tired.

-You've got to be tired.

0:07:160:07:21

Yes, indeed, as Zac's mum and dad are finding out,

0:07:220:07:25

becoming a parent is a tiring business.

0:07:250:07:28

With the snow - for the time being, anyway - melting away,

0:07:300:07:33

Sian Maynard has hit the road.

0:07:330:07:35

But the way it's hitting back is not all that good

0:07:350:07:39

for her blood pressure.

0:07:390:07:41

God. There's a dip there, hold on.

0:07:410:07:43

Oh!

0:07:460:07:47

This is the bit that people don't see,

0:07:490:07:51

my stress now is 10 out of 10.

0:07:510:07:55

Give me a baby to deliver any day.

0:07:550:07:56

This is her first get-to-know visit to farming wife Mari Phillips.

0:07:570:08:01

But even at this early stage,

0:08:010:08:04

there's one problem which she must draw attention to - toxoplasmosis.

0:08:040:08:08

It's usually an infection which is carried with stillborn lambs or

0:08:080:08:12

contact with placentas. But it is general hygiene.

0:08:120:08:15

He wears overalls, they all come off, he washes and changes them

0:08:170:08:20

before he comes anywhere near me.

0:08:200:08:22

-That's it.

-He does his own washing. Yeah.

0:08:220:08:25

-That's even better.

-He does really good washing, actually.

0:08:250:08:28

We've invented this, actually, it's not really true!

0:08:280:08:30

Oh, gosh!

0:08:350:08:38

Sian and her team are happy for Mari -

0:08:380:08:40

because of her age and good health - to have a homebirth.

0:08:400:08:43

But it's the challenge of getting there that's on her mind at the moment.

0:08:430:08:46

We just need to be mindful that we record that it takes 15 minutes

0:08:460:08:51

if you're coming in the middle of the night and it's a midwife from

0:08:510:08:56

another team, just so they have some idea in their head how long the track is.

0:08:560:09:00

From the remoteness of Mari's farm near Cynwil Elfed, Sian's next

0:09:010:09:05

call takes her to the metropolis of Newcastle Emlyn, a town which is

0:09:050:09:09

a good 35 minutes from the maternity unit at Glangwili, Carmarthen.

0:09:090:09:13

Hello, Kelly, how are you?

0:09:130:09:15

Like Mari, 29 weeks pregnant Kelly Walters

0:09:150:09:18

and husband Darren also want a homebirth.

0:09:180:09:22

As this will be Kelly's fourth child,

0:09:220:09:24

Sian doesn't expect complications.

0:09:240:09:26

Even so, things might not go as planned.

0:09:260:09:29

We view each pregnancy as being full-term from

0:09:290:09:33

-when you are 37 weeks pregnant onwards.

-Right.

0:09:330:09:37

So if you happen to go into labour before that,

0:09:370:09:41

then we would advise that you go into hospital to have the baby.

0:09:410:09:45

-Just because the baby will be premature.

-Yep.

0:09:450:09:48

-The pregnancy so far has been perfectly normal.

-Yes.

0:09:480:09:52

So unless anything dramatic happens

0:09:520:09:55

or changes, then we are aiming for a homebirth.

0:09:550:09:58

One baby in every 25 in Carmarthenshire is born at home,

0:09:580:10:03

a figure well above the Welsh average.

0:10:030:10:06

In the 1970s and 1980s

0:10:060:10:07

the Health Service tended to discourage homebirths.

0:10:070:10:11

But attitudes have changed and they're now on the rise again.

0:10:110:10:14

Darren is a songwriter.

0:10:140:10:17

His latest number is a celebration of the baby-to-be.

0:10:170:10:22

HE SINGS

0:10:220:10:26

I'm actually hoping to sing it for my wife when she is in labour.

0:10:260:10:30

Cheer her up a bit and see how it goes.

0:10:300:10:32

I'm writing it for my baby, so it makes sense, really.

0:10:320:10:37

SINGING CONTINUES

0:10:370:10:41

It's very sweet, quite flattering,

0:10:410:10:43

it will be nice to listen to him singing.

0:10:430:10:46

The new baby is due in the heart of winter.

0:10:460:10:49

So there's just one thing troubling Darren.

0:10:490:10:52

If it snows and you can't get here?

0:10:520:10:54

First of all, ring us,

0:10:540:10:57

but you must make sure you've got lots of warm towels.

0:10:570:11:00

If your waters go and you think Baby is coming,

0:11:000:11:04

as the head is being delivered,

0:11:040:11:06

really, just be hands off, baby will actually turn.

0:11:060:11:10

It'll go perhaps from looking at the floor

0:11:100:11:14

-to turning to the left or right.

-Yeah.

0:11:140:11:17

Gently put your hands by the side of the baby's head

0:11:170:11:19

and just pull down gently, and with the pushing as well,

0:11:190:11:24

baby will fall out.

0:11:240:11:25

-That bit, is that with towels in hand or is that just bare hands?

-Up to you.

0:11:250:11:29

The important thing is when a baby is delivered,

0:11:290:11:33

you dry the baby quickly.

0:11:330:11:35

-Really dry them off well and put a warm dry towel around the baby.

-OK.

0:11:350:11:39

-I hope this doesn't happen.

-It won't happen.

0:11:420:11:45

Before all this, I should have said, put your guitar down!

0:11:450:11:49

It'll be your fault...

0:11:490:11:50

Near Llandyfri,

0:11:520:11:54

a midwife is on her way to a scheduled call on a new mother.

0:11:540:11:57

But there was nothing scheduled about the last time Nan Duncan called here.

0:11:580:12:03

Right... We are going to see a little girl

0:12:030:12:07

who had an emergency Caesarean section,

0:12:070:12:11

week before last.

0:12:110:12:13

She is due for a general check-over to see how things are going.

0:12:130:12:19

But it was a hairy moment, I can tell you.

0:12:190:12:22

The "hairy moment" came when Lisa's placenta broke away from her womb.

0:12:240:12:29

Suffering terrible stomach pains and losing a lot of blood,

0:12:290:12:33

something had to be done -

0:12:330:12:35

quickly.

0:12:350:12:36

The first person who came into my head was Nan,

0:12:360:12:38

because I knew who Nan was,

0:12:380:12:40

she knew me, she knew my previous pregnancies.

0:12:400:12:43

Nan said, "I'm coming round straightaway.

0:12:430:12:46

"Ring the ambulance," and that was it.

0:12:460:12:48

It was like, "No, we can't stop and collect this that or anything,

0:12:480:12:52

"we've got to go now."

0:12:520:12:54

That is why we are here. They know us and we're the first person they call.

0:12:540:12:59

It's our job to make sure that the problem gets sorted.

0:12:590:13:02

I remember the surgeon speaking to me and saying,

0:13:020:13:05

"Well, the baby only had minutes, you would've had, like, half an hour

0:13:050:13:09

"to an hour if things were left, and we could've lost you as well."

0:13:090:13:14

One, you were bleeding

0:13:140:13:15

and, two, he was early. We could've lost the two of you.

0:13:150:13:18

-Thank goodness we don't get them every day!

-I know.

0:13:180:13:22

I should be a couple of stone lighter if there was.

0:13:220:13:26

Jayden was born eight weeks prematurely.

0:13:300:13:33

He's still in Glangwili Hospital's Special Care Unit.

0:13:330:13:36

Slowly, he's gaining.

0:13:360:13:39

But he's still too small and too weak to be allowed home -

0:13:390:13:42

a day that won't come soon enough for Lisa.

0:13:420:13:45

We're just waiting to get his feeding established.

0:13:450:13:50

As soon as that's established, sometime in May, he can come home.

0:13:500:13:53

Glangwili Hospital is HQ for the 40 community midwives that

0:13:580:14:02

care for mums in the Carmarthenshire team.

0:14:020:14:05

And on its maternity ward,

0:14:050:14:07

the big day has arrived for first-time mother Bethan.

0:14:070:14:10

This will be a Caesarean delivery,

0:14:100:14:12

and, as it's twins, both Anwen and Sian will be present -

0:14:120:14:17

if Anwen can get over her little phobia, that is.

0:14:170:14:20

I don't like doing this because it's oxygen,

0:14:200:14:23

and it just shoots out sometimes and I'm scared it'll hit my face.

0:14:230:14:27

Oh!

0:14:270:14:29

Not too bad.

0:14:290:14:31

All dressed, ready to go. Right.

0:14:320:14:36

This is obviously Bethan's first pregnancy.

0:14:410:14:43

And she's having twins, which is huge,

0:14:430:14:45

so she is obviously very nervous about the whole process,

0:14:450:14:49

so it would be nice to just get on with it and see the little babies and

0:14:490:14:53

make sure everything is fine, and Bethan's OK after it as well.

0:14:530:14:56

In 2013, over a quarter of all births in Wales

0:14:580:15:02

were by Caesarean section.

0:15:020:15:04

Commonplace it may be, but the surgery involved is major.

0:15:040:15:08

Complications can still happen. But, hopefully, not today.

0:15:080:15:13

45 minutes later, Anwen has the all-important news.

0:15:170:15:21

We've got a boy and a girl. Fantastic! I can weigh them now.

0:15:210:15:26

Now that it's all over, the atmosphere is truly relaxed.

0:15:270:15:31

Whilst Bethan recovers,

0:15:310:15:33

the arms that are used to cradling lambs are now holding babies.

0:15:330:15:37

Look at you! Old hand!

0:15:370:15:39

Easy, wasn't it?

0:15:430:15:45

Easy?! THEY LAUGH

0:15:450:15:47

The little boy came first. Cian David.

0:15:490:15:52

And then, Elliw, followed after, the little girl.

0:15:520:15:56

Now the work begins.

0:15:560:15:58

Oh, yes!

0:15:580:16:00

As the double birth brings double joy to the parents,

0:16:000:16:04

the midwives face the challenge of double the paperwork.

0:16:040:16:07

Sian's getting confused.

0:16:070:16:09

-Sian can't count.

-Very confused.

0:16:090:16:12

For midwives, working days often become working nights.

0:16:120:16:16

The end of the shift has come and gone.

0:16:160:16:19

And so - as often happens - it's now phone-a-friend time.

0:16:190:16:24

I'm lucky that my friend Cheryl has managed to get hold of her

0:16:240:16:27

mother-in-law and she's picked up my children for me from school.

0:16:270:16:30

And hopefully take them back to her house

0:16:300:16:34

till I get back and pick them up.

0:16:340:16:37

Finally, the moment that's been a long time coming.

0:16:390:16:43

Following an emergency Caesarean and 12 days of very special care,

0:16:430:16:48

Lisa and little Jayden are homeward bound.

0:16:480:16:51

Together, at last.

0:16:510:16:54

Jayden has been let out today.

0:16:540:16:56

We'll be going home.

0:16:560:16:59

Can't wait.

0:16:590:17:01

Taking a baby home brings excitement,

0:17:040:17:08

but also worries.

0:17:080:17:09

This is why, today, Nan is on her way to see Lisa.

0:17:090:17:12

She's had no trouble breastfeeding her previous children.

0:17:140:17:18

But, a day after getting Jayden home, Lisa isn't happy.

0:17:180:17:22

I got a breast pump.

0:17:220:17:24

I used it yesterday

0:17:240:17:26

and I used it this morning

0:17:260:17:28

and I've only got, like, 20ml on each one...

0:17:280:17:33

Not that much at all.

0:17:330:17:35

Breastfeeding may be natural.

0:17:350:17:38

But it's something that has to be worked on,

0:17:380:17:40

and mothers often need extra support.

0:17:400:17:43

With all her years of experience, Nan just knows what's wrong

0:17:430:17:47

and what to do about it.

0:17:470:17:49

It's instinctive, really, it's like being a detective, the midwife knows

0:17:490:17:52

the parameters of normality and who to call and how to make the outcome safe.

0:17:520:17:57

He needs to have the quality and quantity of feed

0:17:570:18:01

because we don't want his weight to dip down.

0:18:010:18:05

That's when his energy levels are sapping and we'll have a big problem.

0:18:050:18:09

So you've got to show him how to get onto that breast properly.

0:18:090:18:14

Behind the tips, you have got a reservoir...

0:18:140:18:18

Nan shares her knowledge so that little Jayden may get

0:18:180:18:22

the sustenance that he so badly needs from his mother's breast.

0:18:220:18:25

You want some winding as well to go with it...

0:18:250:18:28

To parents with their doubts and anxieties, the calm reassurance

0:18:280:18:31

and good-natured advice of the experienced midwife is invaluable.

0:18:310:18:35

So I need you to eat loads.

0:18:350:18:37

-Is that very important, to eat?

-Huge amount, huge.

-Oh, is it?

0:18:370:18:42

Huge!

0:18:420:18:44

Show me now how you'd feed him.

0:18:450:18:48

Right, that's it.

0:18:480:18:50

When his mouth is open wide, you can whack him on.

0:18:500:18:54

You want to develop my whack.

0:18:540:18:57

The all-seeing eyes watch carefully as Lisa manages to satisfy

0:18:590:19:03

little Jayden's need.

0:19:030:19:05

Home at last.

0:19:050:19:07

The twins are also home and, with the help of student

0:19:100:19:12

midwife Natalie, Anwen is paying her final home visit to Bethan

0:19:120:19:16

and Dylan before passing care on to the health visitor.

0:19:160:19:21

-So, eating OK, both of them?

-Yeah, both of them.

0:19:210:19:23

We check the eyes, we check that the mouth is nice

0:19:230:19:28

and clear to make sure there are no signs of thrush or anything.

0:19:280:19:31

We check the colour of the babies, make sure they're nice and pink,

0:19:310:19:34

no jaundice.

0:19:340:19:36

We then check the umbilical cord to make sure that's OK.

0:19:360:19:39

We always check if there's wet and dirty nappies so that we know

0:19:390:19:43

the babies are feeding fine,

0:19:430:19:44

their urine and their bowel movements are all fine.

0:19:440:19:47

Anwen's journey with this family is coming to an end.

0:19:530:19:56

Today will mean breaking the umbilical cord of care

0:19:560:19:59

and friendship between her and Bethan and the babies.

0:19:590:20:02

You do get attached to the babies - how can you not?

0:20:040:20:07

They are absolutely beautiful.

0:20:070:20:10

But with Bethan and Dylan, they are up a lot in the night -

0:20:100:20:12

at least I get to sleep at night.

0:20:120:20:14

THEY LAUGH

0:20:140:20:17

It's a lot of hard work, but between the two of us,

0:20:170:20:19

we get round it in the end, I think.

0:20:190:20:21

I am going to miss Anwen, it is really nice to have somebody here.

0:20:210:20:26

She's been a lot of help and support to me over the last few months.

0:20:260:20:30

It can be really emotional

0:20:310:20:32

because you get to know the mothers during the pregnancy.

0:20:320:20:35

You do build up a relationship with them.

0:20:350:20:38

It is going to be strange after today, not to have them around.

0:20:400:20:46

Both absolutely healthy, perfectly healthy twins.

0:20:460:20:49

-Ta-ra!

-Ta-ra!

-Ta-ra!

0:20:510:20:54

Oh, 'eck! Oh, dear.

0:20:560:20:59

Erm...have you got a jack?

0:20:590:21:03

The tyre's flat, but Dylan is buoyant as he enjoys this

0:21:030:21:08

opportunity to show his gratitude.

0:21:080:21:10

Aw, thank you, Dylan.

0:21:100:21:12

And away they go, but not to the nearest garage.

0:21:120:21:16

It's on to the next new mother and the next new baby.

0:21:160:21:19

Midwives enjoy their work.

0:21:240:21:27

But even the most dedicated of midwives such as Nan Duncan

0:21:270:21:30

have to have time to themselves.

0:21:300:21:32

Come on, then! Come on!

0:21:320:21:35

My husband's hobby, nothing to do with me, I just put up with him.

0:21:350:21:38

I just pay for him!

0:21:380:21:40

I wish.

0:21:400:21:43

It's a beautiful place to live and completely different to my work.

0:21:430:21:47

-I can completely unwind here.

-Come on.

0:21:470:21:52

Over 33 years as a midwife, Nan's built up a wealth of knowledge

0:21:520:21:55

and expertise.

0:21:550:21:58

But it's only recently that's she come to question, at what cost?

0:21:580:22:01

Three children, a farm, a business.

0:22:010:22:05

Yes, they have lost out a lot.

0:22:050:22:08

It's taken me until this age, really,

0:22:080:22:11

to realise that the important things in life is your family.

0:22:110:22:15

My grandchild now, I've got far more time with him

0:22:150:22:18

because I realise what I'd lost with my own children.

0:22:180:22:21

This is my husband, Wayne. Sometimes known as Ginge.

0:22:240:22:27

But he's turning white now so I ought to call him Whitey now, I suppose.

0:22:270:22:31

With rural midwives often on-call day and night,

0:22:330:22:36

should Wayne perhaps be called a "mid-husband"?

0:22:360:22:39

He's good, very good, actually.

0:22:420:22:43

Although I don't give him credit to his face,

0:22:450:22:47

-he is extremely good.

-I can't see me

0:22:470:22:50

part-exchanging her for a nice 18-year-old,

0:22:500:22:53

but that was the idea in the past,

0:22:530:22:55

but I think we've gone past that now.

0:22:550:22:58

It's all good fun and good stuff -

0:22:580:23:00

it is what makes marriage and a happy life, isn't it?

0:23:000:23:03

In the heart of the Teifi valley, midwives Anwen and Sian

0:23:050:23:08

are not hanging about, as they head for Newcastle Emlyn.

0:23:080:23:12

Darren has had to put down his guitar and pick up the phone.

0:23:150:23:18

Things are beginning to move.

0:23:180:23:19

Darren rang to say Kelly had started having contractions.

0:23:190:23:23

-I could hear her. It was the end of one contraction, wasn't it, Darren?

-Yeah.

0:23:230:23:27

I thought, she is cracking on, so it was...running out into the car park.

0:23:270:23:32

Darren and Kelly already have three boys.

0:23:320:23:36

Knowing that the imminent birth will give them a baby girl

0:23:360:23:39

is making it difficult for Darren to contain his excitement.

0:23:390:23:42

Kelly will tell you, I am terrible at waiting.

0:23:420:23:45

We were expecting, through previous births, for her to be early,

0:23:450:23:49

but she is on her due date so exciting times.

0:23:490:23:52

Feeling quite good at the minute, relaxed...

0:23:540:23:57

So the contractions are coming quite regularly now.

0:23:570:24:01

We now monitor how often Kelly is having the contractions.

0:24:010:24:06

We see how long they are lasting.

0:24:060:24:09

We listen in to baby's heartbeat

0:24:090:24:11

to make sure the baby is coping well with

0:24:110:24:14

the contractions as well.

0:24:140:24:17

It's Kelly's fourth baby

0:24:170:24:19

and she's had others no problem, so no concerns or anything with Kelly.

0:24:190:24:24

Just go with it now and hopefully have a lovely baby pretty soon,

0:24:240:24:28

for Kelly's sake, isn't it?

0:24:280:24:30

For midwives,

0:24:330:24:35

"pretty soon" can in fact mean quite a few hours, as the stages of birth

0:24:350:24:39

progress according to the mother's and the baby's very own timetable.

0:24:390:24:43

Go with your body now, OK?

0:24:460:24:49

Nice, slow breathing.

0:24:490:24:52

Watching carefully is an important part of Anwen

0:24:520:24:55

and Sian's work throughout the whole pregnancy.

0:24:550:24:59

Despite the relaxed atmosphere that they are purposefully

0:24:590:25:01

maintaining, it's in these last critical hours

0:25:010:25:05

that their watchfulness becomes hyper-sensitive.

0:25:050:25:08

DEEP BREATHING

0:25:080:25:10

Well done.

0:25:100:25:12

GROANING

0:25:120:25:14

That's right.

0:25:140:25:16

You're doing brilliantly. Wash the pain away.

0:25:160:25:19

SHE INHALES AND GROANS

0:25:190:25:21

That's the way.

0:25:210:25:23

Even though this is Kelly's fourth birth, it's no easier...

0:25:230:25:26

and the painful, contraction-after-contraction

0:25:260:25:28

process of giving birth

0:25:280:25:30

draws mother, father and midwives closer and closer together.

0:25:300:25:34

Slowly now...

0:25:340:25:37

-I've got cramp in my blinking leg!

-Which one...?

0:25:370:25:40

-Left.

-Yours!

0:25:400:25:43

I see her now, she's on her way. All signs looking good.

0:25:460:25:50

Despite Kelly's obvious discomfort, everything is looking good.

0:25:500:25:54

The miraculous moment of birth is imminent.

0:25:540:25:57

A little hand coming there as well.

0:26:000:26:03

A really big push now.

0:26:030:26:05

-Well done.

-Here she is!

0:26:050:26:07

BABY CRIES

0:26:070:26:10

Ohhhhh!

0:26:100:26:12

There we are! Fantastic!

0:26:120:26:14

And so it happens.

0:26:140:26:17

Zoe Jane is born.

0:26:170:26:20

Safe and sound.

0:26:200:26:22

She's fine.

0:26:220:26:24

-Is she a girl?

-She's definitely a girl. She's gorgeous.

0:26:240:26:30

SHE SOBS

0:26:300:26:32

Yep...there we are.

0:26:320:26:36

It's relief and joy all round, of course.

0:26:370:26:41

But, although the hardest work is done,

0:26:410:26:43

the placenta - the afterbirth - has still to be delivered.

0:26:430:26:46

A nice and healthy one.

0:26:460:26:49

Saving and serving it up for its health benefits is a growing trend amongst parents,

0:26:490:26:54

although few, perhaps, are as enthusiastic as Darren.

0:26:540:26:57

I've managed to get one of my chef friends to agree to cook it.

0:26:570:27:00

So, it should go in the freezer, and be ready to be eaten.

0:27:030:27:06

-Hello.

-Another good day's work, Anwen?

0:27:080:27:12

A perfect day's work.

0:27:120:27:14

-Lovely, and I've got two days off now so I'll be on a high now.

-Yeah.

0:27:140:27:18

It's very painful...

0:27:190:27:21

and, er, I'm glad she's here now.

0:27:210:27:25

Finally, whilst Anwen and Sian

0:27:270:27:29

get to grips with all the boxes that need to be ticked,

0:27:290:27:33

Darren greets his newborn daughter with a newborn song.

0:27:330:27:37

# Sunshine of my life The twinkle in my eye

0:27:370:27:43

# You're everything to me... #

0:27:430:27:48

You just look at them there now and just think,

0:27:480:27:51

this is what it's all about.

0:27:510:27:54

# It's plain for all to see

0:27:540:27:58

# Why you are everything to me. #

0:27:580:28:01

Another hard day's night draws to a happy close.

0:28:010:28:05

I know you're not allowed to drink, so I have got some apple juice

0:28:050:28:08

-and lemonade.

-Thank you.

-Congratulations!

0:28:080:28:11

Well done, Kelly, this is to you and our beautiful little girl, Zoe Jane.

0:28:110:28:14

-Yeah.

-Well done.

-Congratulations.

0:28:140:28:17

Next time on The Country Midwives...

0:28:200:28:22

Anwen's friendly call becomes a call for concern...

0:28:220:28:26

Sian hears about the strangest of cravings...

0:28:280:28:30

Flannels. Just sucking flannels.

0:28:300:28:33

And carnival star Lynne delivers a show-stopping home-birth.

0:28:330:28:36
