Lisa Norris, who was given an overdose in 2006, ten years ago. She | :09:50. | :09:54. | |
endured terrible suffering before her death. The overdose to this | :09:55. | :10:00. | |
patient was even higher than that, but in other ways the circumstances | :10:01. | :10:04. | |
were different. The Lisa Norris case happened because of faults in the | :10:05. | :10:09. | |
system, and in the wake of that case, lots of changes were brought | :10:10. | :10:14. | |
in to make the system safer. In this case, individuals got the sums | :10:15. | :10:18. | |
wrong. Several individuals all got the sums wrong, but it seems to be | :10:19. | :10:22. | |
down to individual error. The computer system did flag up that | :10:23. | :10:27. | |
they had made a mistake, and they manually overrode it. As a result of | :10:28. | :10:32. | |
an investigation into this case, it has been recommended that staff are | :10:33. | :10:35. | |
retrained in the computer system so they have more confidence in it. It | :10:36. | :10:39. | |
has also been pointed out that it happened over a weekend, so NHS | :10:40. | :10:42. | |
Lothian have beefed up on-call cover at weekends. We do not know what | :10:43. | :10:47. | |
happened to the patient involved, because they are anonymous and | :10:48. | :10:51. | |
entitled to confidentiality. However, we do know it was | :10:52. | :10:55. | |
palliative, so it was end of life treatment. It is possible that this | :10:56. | :10:58. | |
patient actually passed away before they felt the ill effects. | :10:59. | :11:01. | |

people in Scotland to remain in EU membership, that is an issue of | :12:40. | :12:42. | |
significant concern that has to be addressed. What we are doing, the | :12:43. | :12:47. | |
Scottish Government, is acting with all of our energy to protect | :12:48. | :12:51. | |
Scotland's EU membership, to make sure the vital interests of the | :12:52. | :12:54. | |
agricultural sector are protected as part of that process. Indeed, Brexit | :12:55. | :12:59. | |
negotiations and a future funding deal are in the hands of the UK | :13:00. | :13:04. | |
Government. Today, they said the Prime Minister was clear that there | :13:05. | :13:07. | |
needs to be a system of agricultural support, in the event of a decision | :13:08. | :13:11. | |
to leave the EU. The Government will work with industry and the public to | :13:12. | :13:16. | |
develop these new arrangements. The decision by voters to leave the | :13:17. | :13:20. | |
European Union still reverberate around this land, two weeks on. | :13:21. | :13:26. | |
Farmers are used to uncertainty, but perhaps this is one factor they did | :13:27. | :13:27. | |
not bargain for. Inspectors who've visited Aberdeen | :13:28. | :13:31. | |

life. His father says what he is hearing today is nothing short of | :14:34. | :14:37. | |
offensive. It is a complete insult, rubbing salt in the wounds when | :14:38. | :14:41. | |
there does not need to be any of it. It is hard enough as it is. Parents | :14:42. | :14:45. | |
are in a difficult situation, hurting enough as it is. With the | :14:46. | :14:49. | |
report that has come out, to hear this that they are the victims, is | :14:50. | :14:56. | |
unbelievable, really. Last week's inspection hollered a damning report | :14:57. | :14:59. | |
into the running of this crematorium. But this union leader | :15:00. | :15:04. | |
says it is the report that describes them as victims, not the workers | :15:05. | :15:09. | |
themselves. I need to be clear that no member of staff has come forward | :15:10. | :15:13. | |
and said, you know, I feel that way, I feel that we are the victims. In | :15:14. | :15:22. | |
