A failure in preventing infections was the most likely cause of an

E-coli outbreak at a hospital where two premature babies died after

contracting the bug, a health watchdog has found. The outbreak at

Swansea's Singleton Hospital involved a mother, her twins and a

third baby in November 2011. The neo-natal unit was shut for about a

month following the outbreak. Here's our reporter, Cemlyn Davies.

The units here was very busy when the incident happened three years

ago. There was a shortage of carts. The Inspectorate also said the fact

that these two sick babies had been placed so close to one another was

at significant factor in this case. The Inspectorate said it was the

exact cause of the transmission and the outbreak. This was most likely

caused by a failure in infection prevention and control within the

neonatal unit. The report then makes a series of recommendations 13 in

total. The report also points out that the health board carried out

its own review into what happened and identified a number of areas

where improvements were required, including the provision of hand

washing and a new form or space between carts. The health board

itself says it's newly refurbished unit now has a special isolation

room, which it hopes will reduce -- reduce the risk of cross infection

in future. One in five Welsh households has

no-one working. A new scheme is being launched aimed at breaking

this cycle. 5,000 young people, from eight parts of Wales, will be given

work experience as part of the Lift programme. Our economics

correspondent, Sarah Dickins, has been to Blaenau Gwent, the worst

affected area. Here they are as they arrive. Out of

work for more than six months and with a history of turbulent homes

and prison. This is them now, six weeks on, working like fire

officers. Smart, discipline, fitter, and working as a team. Wade

left school before taking any exams to look after his grandfather. Here

they have got firefighters and people from the higher ranks giving

you a chance and giving you an opportunity to better yourself. I

never had that before. Nobody has ever given me a chance for -- a

chance. Divided by the permanent staff here, this course has not been

about giving people a job at changing their lives. -- are

developed. But like firefighters, they have to train for half an hour

per day in the gym to give them more strength, confidence and energy.

In the fire station, cooking and eating together is an important part

of teamwork. These young people are learning to live more healthily,

too. At one time, this girl was sleeping rough after taking drugs

took hold of a lie. Now she is preparing for university. I can't

even blame society. It is so easy to get that stuff around here. They

didn't have much ambition. If they had been stuck in this particular

frame of mind, long-term unemployed with their background, it is easy to

get lost in that. So we have given them something positive. As a

group, they have gained experience to impress possible employers. The

challenge remains to find a job. Steps need to be taken to help them

along the road to getting work. But that is only part of the story.

There is still a massive challenge in terms of the numbers of jobs and

the quality of jobs available. Their six weeks over, the colleagues were

given a formal sendoff in front of their parents. Somebody once told me

that the kindest people are the ones who come into your life and show you

the sunshine between the clouds, the ones who believe in you. Their final

chance to show off their firefighting skills. The next step

is to use their new confidence to get the job. A real challenge in the

communities they are from. A homeless man has appeared before

magistrates accused of murder after a body was found in the River Taff

in Cardiff. David Alun Lewis, from Ystrad Mynach, was found in the

water near the Millennium Stadium. Gareth Wyn Jones has been remanded

in custody and is due to appear at Cardiff Crown Court on Wednesday.

Gwent Police say no further action will be taken against three men

arrested last year on suspicion of slavery. The three, who are aged 53,

38 and 20, were arrested in St Brides on the outskirts of Newport

in September last year, but they have never been charged.

The Prime Minister's being asked to prevent public money being given to

a proposed new motor racing track in Blaenau Gwent. Bosses at the

Silverstone racetrack have written to David Cameron claiming that the

?50 million of taxpayer money requested by the Circuit of Wales

breaches European rules. Here's our business correspondent, Brian

Meechan. MotoGP started in Qatar this week.

It reaches around 300 million people across the globe. That is an

audience that those behind the circuit of Wales want here. This is

the undeveloped site at the moment. The company says it has the backing

for most of the project but needs ?30 million from the Welsh

government and ?20 million from the UK government to go ahead. The man

behind the circuit denies Silverstone's claims that this

estate would be illegal under EU rules. If anything, there is anger

and disappointment that what we are proposing to do is a major

investment in one of the poorest part of our country that seems to be

deliberately being put at risk by competitors for their own interests.

De MotoGP is currently held at Silverstone and it is bidding again

to host the event. Those behind the circuit of Wales have been accused

of exaggerating the economic benefit. Other tracks operating

tight circumstances. This one made a profit of more than ?800,000 in

2012. In the same year, Dunnington made a loss. Silverstone's losses

topped ?3 million. They are more concerned about a level playing

field, about the fact that the Welsh government is putting money into the

circuit and that is money that the likes of Silverton have not had.

Jobs and prosperity for one of the most deprived parts of the country

is the hope. But there are plenty of questions over money that need

answering before the circuit of Wells gets the green light. -- the

circuit of Wales. Rugby, and Wales' captain Ryan Jones

is leaving the Ospreys. The 33-year-old, who's been with the

Welsh region for ten years, has signed a two-year deal with Bristol.

He becomes the third Welsh player to sign for the English side. He joins

Osprey's team-mate Matthew Morgan and Sale scrum-half Dwayne Peel.

cold as last night and the temperature was -4 in Snowdonia.

Tomorrow I can promise you some sunshine but it is unlikely to stay

dry everywhere will they. Tomorrow starts off damp in the north-east.

The rain with clear away. Elsewhere, Mitchell cloud and sunshine, perhaps

the odd shower. Across the rest of the UK, band of rain lying through

England into Scotland will move westwards during the day. It will

brighten up in the South West. Some sunshine developing, too, in East

Anglia. A few showers for Northern Ireland. Chilly and damp for London

tomorrow. We may see some showers in Cardiff by the middle of the

afternoon. Tomorrow evening and through the afternoon, some patchy

rain and showers. It should stage I and bright in Swansea. Tomorrow

evening and overnight, there will be some showers over Wales. Trying up

in the north-east, with a ground frost. A few showers first thing on

Wednesday. These will clear. More sunshine and showers in the

afternoon. So, mixed weather this week. Sunny spells and showers. A
