week? Good evening. A failure to prevent infections was the most

likely cause of an ecoli outbreak at a Swansea hospital where two newborn

:01:24.:01:26.

babies died. That's the conclusion of the watchdog, Health Inspectorate

:01:27.:01:35.

Wales. It reviewed the incident at Singleton Hospital three years ago.

:01:36.:01:38.

Our reporter Cemlyn Davies is there for us tonight. What is the

:01:39.:01:48.

background to this? Well this goes back to the autumn of 2011 when a

:01:49.:01:53.

pair of twins and a third baby were born at the hospital prematurely and

:01:54.:01:56.

all three needed neonatal intensive care. The twins' mother had

:01:57.:01:59.

contracted E coli, which then developed in her new born babies.

:02:00.:02:03.

Tragically one of the twins died and the third baby - who was being cared

:02:04.:02:07.

for in the next incubator - also passed away after contracting the

:02:08.:02:14.

infection. Tests carried out at the time found one of the babies had

:02:15.:02:18.

caught E coli within the unit. You remember, the neonatal unit was

:02:19.:02:28.

closed for about a month. And what has the health inspectorate found?

:02:29.:02:34.

Well according to the inspectorate's report the neonatal unit was very

:02:35.:02:38.

busy at the time and there was a shortage of cots. The inspectorate

:02:39.:02:41.

says the fact two sick babies were placed next to each other was a

:02:42.:02:44.

significant factor in this case. The inspectorate says that, whilst the

:02:45.:02:47.

exact cause of the transmission and the outbreak cannot be determined,

:02:48.:02:51.

it was most likely caused by a failure in infection prevention

:02:52.:02:53.

control within the neonatal unit. The report then makes a total of 13

:02:54.:03:02.

recommendations. And what has the health board had to say about this?

:03:03.:03:08.

The report points out that the health board has carried out its own

:03:09.:03:12.

reviews and found areas where improvements were required. For

:03:13.:03:14.

example the health board recognised there was inadequate provision for

:03:15.:03:18.

hand washing and there was a need to increase the space between cots. The

:03:19.:03:21.

health board also says the newly refurbished neonatal unit here at

:03:22.:03:24.

Singleton Hospital now has a special isolation room to reduce the risk of

:03:25.:03:38.

cross infection in the future. Jurors have been chosen for the

:03:39.:03:41.

trial of a mine manager facing manslaughter charges after the

:03:42.:03:44.

deaths of four miners at a Swansea Valley mine. It was the manager of

:03:45.:03:52.

the mind when it flooded in September 2011. Charles Breslin,

Philip Hill, Gary Jenkins and David Howell all died in the incident. He

denies manslaughter whilst the company which operated the site

universe. A powerful telescope in the South Pole has picked up signals

from space dating back 14 billion years. A telescope designed by
