of murdering the children's author Helen Bailey has told a court

before her disappearance. Helen's fiance Ian Stewart

denies drugging her, killing her and hiding her body

:05:21.:05:25.

The court heard on the day Helen disappeared, she returned, asking,

"Have you remembered anything or found anything

He then whispered, "Just think about it".

Ian Stewart said Nick later returned and said,

Ian Stewart said he believed if he told the authorities,

The judges said Helen Stewart and Ian Bailey had

:06:46.:06:48.

They never had an argument and in 2012 secretly got engaged.

:06:49.:06:52.

They heard that in early 2016, Ian Stewart had been

:06:53.:06:54.

having trouble sleeping and was prescribed a sedative.

:06:55.:06:56.

The prosecution claimed that he drugged his fiancee

:06:57.:06:58.

Asked today if he took the tablets, she said, no.

"Any pushing or pulling was intensely painful.

Asked if he had killed harder for financial reasons, he replied, no.

He said he had more money than he could spend. He denies all the

charges against him and his defence case will continue here tomorrow.

Now, how to persuade more young doctors to train to become GPs.

:08:11.:08:13.

A quarter of our GPs are over 55 and approaching retirement,

:08:14.:08:16.

with surgeries across the East already struggling to cope

:08:17.:08:26.

A new scheme in Bedford is aiming to entice medics into the profession

:08:27.:08:30.

by giving them the chance to do some work as a specialist.

:08:31.:08:33.

Doctor Jazmine Ali, one of the first to sign up.

:08:34.:08:39.

She is able to split her time between the GP rota and building up

:08:40.:08:42.

With this scheme, it gives you a bit of balance between not

:08:43.:08:49.

being expected to be in a GP practice for five days.

:08:50.:08:51.

It gives you that leeway, if you like, to balance out the way

:08:52.:09:01.

that you would want your profession to progress.

:09:02.:09:06.

Here at this surgery, they do have enough GPs.

:09:07.:09:08.

But they hope that the project will help with general recruitment.

:09:09.:09:11.

The question is whether it's worth the trade-off of GPs not working

:09:12.:09:15.

to clinics all week to try and help with recruitments.

:09:16.:09:20.

If, say, 30% stay and really get into it, and become good at it,

:09:21.:09:28.

and then have an interest which they can then develop

:09:29.:09:32.

and become a specialist, that might solve the problem.

There are lots of part-time doctors and I'm quite happy to see,

:09:38.:09:40.

particularly the lady part-time doctors, and if they have

:09:41.:09:42.

While there are national GP shortages, it's even worse

:09:43.:09:53.

They've got 250 GPs now, but still need another 30.

:09:54.:10:01.

I think there will always be a gap, there will always be a tension

:10:02.:10:07.

within general practice because of the way, the way

:10:08.:10:09.

It is no longer attractive to GPs and I think we have a wider,

:10:10.:10:15.

you're quite right, a wider, transformative change in how

:10:16.:10:18.

we deliver our general practice and primary care operations.

:10:19.:10:23.

It is hoped the new scheme will not only help to attract new GPs,

:10:24.:10:26.

So why don't more people want to be GPs?

An expert in primary health care from Cambridge University,

Professor Martin Roland, joined me earlier to explain

Not just this region, there's a shortage of GPs throughout

:10:43.:10:48.

the country and it's a combination of two things.

:10:49.:10:50.

There have been fewer people, fewer young doctors wanting to go

:10:51.:10:53.

into general practice but also a real problem of GPs

:10:54.:10:56.

retiring earlier, around 60 or even their late 50s,

:10:57.:10:59.

because they find their job very stressful.

:11:00.:11:05.

And will this idea of specialisms help to attract young medics

:11:06.:11:07.

The Government is committed to trying to provide

:11:08.:11:12.

There is a range of things they are doing.

:11:13.:11:16.

Giving GPs an opportunity to have a special interest is one

:11:17.:11:23.

of the things that could help, because doctors often have a lot

:11:24.:11:26.

of experience in a particular specialty when they been training

:11:27.:11:28.

and to have an opportunity to do that for part of the time

:11:29.:11:31.

when they are being a GP for the rest of the time can

:11:32.:11:35.

If we look at the specialism of mental health care, say,

:11:36.:11:39.

GP appointments are quite short, aren't they?

:11:40.:11:41.

So how can you really get a picture or a diagnosis of a mental health

:11:42.:11:44.

I think if you had a practice and one of the GPs was taking

:11:45.:11:50.

a special interest in mental health problems and perhaps seeing patients

:11:51.:11:53.

that had been referred by one of the other GPs,

:11:54.:11:55.

I think that practice would arrange for that doctor to have more time

:11:56.:11:58.

Will streamlining the service in this way eventually

:11:59.:12:01.

Potentially some patients who might otherwise be referred to hospital

:12:02.:12:09.

specialists could be seen in general practice and there have been

:12:10.:12:12.

a number of trials, for example, of GPs with a special interest

:12:13.:12:16.

in skin problems, so minor skin problems could be seen in general

:12:17.:12:20.

practice that some GPs might feel they need to refer to a specialist.

:12:21.:12:23.

So there is potential for fewer patients to be referred to hospital.

:12:24.:12:27.

In general, what could attract more people to the profession?

:12:28.:12:32.

It needs to be attractive to medical students,

:12:33.:12:35.

more incentives to young doctors to come and train in general

:12:36.:12:37.

practice and a raft of things too - make the job more doable,

:12:38.:12:40.

less administration, less regulation and more time

:12:41.:12:42.

"Spit hoods" are being handed out to every Thames Valley Police

The hood is put over the heads of offenders to prevent them

spitting at police and the public or biting them.

Critics have described them as cruel and degrading.

I wanted to put something up that personal in their because my parents

dream. For ten years he worked in various jobs but always knew he
