One of Poppi's worn nappies was allowed to be thrown away.

A concern that she may have been seriously sexually assaulted

And witness accounts were not taken until eight months after her death,

At that point, her father, Paul Worthington was questioned

on suspicion of sexual assault, something he denied.

Both parents were later released without charge.

An inquest into her death lasted just seven minutes and recorded

The criminal investigation faded from public view.

But at the beginning of 2016, a family court judge ruled that

Paul Worthington probably did sexually abuse his daughter,

something he has always strongly denied.

The Crown Prosecution service looked at the case again,

but ruled that the police didn't have enough evidence

Today, the Chief Constable of Cumbria Police accepted the IPCC

criticism and apologise to Poppi's family.

We've been working very hard since 2014 to this date,

to try and make sure that we put things in place to make sure

And I'm confident that that is the case.

Police investigating the death of a two-year-old boy in Birkenhead
