An Inquiry into the death of an unarmed man, shot by police,

:00:00.:00:17.

has heard two of the officers failed a firearms course just weeks before.

:00:18.:00:22.

E-mails also revealed concerns about how the officers

:00:23.:00:25.

Anthony Grainger died as he sat in a car in Cheshire in 2012.

:00:26.:00:31.

Our Social Affairs Correspondent Clare Fallon reports.

:00:32.:00:37.

Five years ago last week, in this car park,

:00:38.:00:50.

an unarmed man was shot dead by police.

:00:51.:00:52.

The firearms officers who came here that night

:00:53.:00:54.

should have been highly trained, used to dealing with the most

:00:55.:00:56.

Now, we have learned that two of them had failed

:00:57.:01:00.

a training course in the weeks before.

:01:01.:01:01.

One of them, only known as Z15 had made such serious safety

:01:02.:01:04.

breaches on that training exercise, it could have ended his career as a

:01:05.:01:07.

The other who failed, X7, was the most senior

:01:08.:01:11.

firearms officer here when Anthony Grainger was shot dead,

:01:12.:01:14.

effectively running the operation on the ground.

:01:15.:01:16.

Having heard the details during today's hearing,

:01:17.:01:22.

Anthony Grainger's parter told me that she finds these

:01:23.:01:24.

How difficult is that for you to sit through and hear

:01:25.:01:36.

We want to find out what really happened that night and why Anthony

:01:37.:01:47.

Giving evidence today, Michael Lawler.

:01:48.:01:50.

He is now retired, but at the time, he was head of

:01:51.:01:54.

firearms and heavily involved in planning the Greater Manchester

:01:55.:01:56.

Police operation which ended in Anthony Grainger's death.

:01:57.:02:00.

He told the enquiry that he didn't think the

:02:01.:02:02.

police force knew that they had felt that course until after the event

:02:03.:02:10.

and that he had destroyed a book containing some of his notes about

:02:11.:02:14.

the operation when he left the police force.

:02:15.:02:16.

The enquiry is still to hear from the police

:02:17.:02:21.

The public hearing is scheduled to last several more weeks.

:02:22.:02:34.

Liverpool Crown Court heard an elderly woman fell

:02:35.:02:37.

on to a railway track after a Merseyrail guard failed

:02:38.:02:40.

to check all passengers had boarded safely

:02:41.:02:41.

Martin Zee denies endangering passenger safety.

:02:42.:02:45.

Edna Atherton was 88 when she fell from the platform,

:02:46.:02:50.

suffering broken ribs and a cut head at Birkenhead's

:02:51.:02:52.

Northern Rail says it'll only be able to operate 40%

:02:53.:02:59.

of services next Monday, when the RMT union stages a one day

:03:00.:03:02.

strike in protest at plans to introduce driver-only trains.

:03:03.:03:07.

Police have bailed the 89-year-old driver,

:03:08.:03:10.

who was arrested after two women were killed by a car

:03:11.:03:13.

outside Withington Hospital in Manchester yesterday.

:03:14.:03:15.

The man was questioned on suspicion of causing death

:03:16.:03:17.

The women, both in their 40s, have not yet been named.

:03:18.:03:27.

A woman from Morecambe has been told she can't have a drug

:03:28.:03:29.

Last summer, Harriet North was diagnosed with a rare terminal

:03:30.:03:33.

The treatment is almost completely effective,

:03:34.:03:38.

but the NHS only offers it to patients diagnosed

:03:39.:03:40.

Music means everything to Harriet North.

:03:41.:03:49.

But due to her condition, playing guitar now means

:03:50.:03:51.

I know that is going to flare-up within the next few days.

:03:52.:04:00.

confirming she wouldn't be prescribed the drug, Anakinra, which

:04:01.:04:11.

The following CCU criteria has not been met.

:04:12.:04:14.

Just tell us how worrying a time this is for you.

:04:15.:04:17.

Especially with specialists saying that in ten years' time, I'm going

:04:18.:04:23.

to get kidney failure, liver failure and my system

:04:24.:04:25.

If I don't get the drug that I need, I won't be here in ten years.

:04:26.:04:33.

A change in NHS rule means that he she can't have

:04:34.:04:36.

Anakinra, even though as ?10,000 a year,

:04:37.:04:38.

it is cost-effective and doctors say it is 95% effective

:04:39.:04:40.

In March last year, NHS rules changed, meaning any

:04:41.:04:46.

drug that is not approved by Nice can no longer be prescribed to new

:04:47.:04:50.

To get Nice approval, a drug needs to be tested

:04:51.:04:53.

Because there is only one in a million of us

:04:54.:04:57.

and there is so little research on it, when they do find

:04:58.:05:00.

in the research and find a drug that works, it is frustrating

:05:01.:05:03.

that the NHS don't back that drug.

:05:04.:05:07.

The worrying thing as a parent is going forward.

:05:08.:05:09.

She mentions ten years, I think that is an outside hope.

:05:10.:05:13.

The family say they have been overwhelmed by the support that

:05:14.:05:16.

Harriet has received with friends offering to stage

:05:17.:05:18.

We would struggle to fund it ourselves.

:05:19.:05:21.

It would cost them a lot more to keep them alive on dialysis,

:05:22.:05:27.

They are now hoping that NHS England will overturned the decision to

:05:28.:05:37.

Harriet's MP David Morris is supporting her challenge

:05:38.:05:45.

My office has been dealing with Harriet's consultants and they have

:05:46.:05:52.

said that she is the worst case out of 70 in the country.

:05:53.:05:58.

NHS England deem her not fit to have this

:05:59.:06:00.

drug and it is quite frankly not good enough and I am fighting this

:06:01.:06:04.

When you meet with Jeremy Hunt next week, what are going to say?

:06:05.:06:08.

I am going to enlist Secretary of State's help to

:06:09.:06:10.

his neck out and make an exception in one individual case, isn't it?

:06:11.:06:18.

That could for him open the floodgates.

:06:19.:06:20.

At the end of the day, the criteria is there.

:06:21.:06:23.

This drug is available to those who really need it.

:06:24.:06:27.

Are you optimistic you will get this turned around for her?

:06:28.:06:31.

I never promise anybody anything, but one thing I do promise

:06:32.:06:34.

and one thing I will always deliver is a good fight and I normally get

:06:35.:06:38.

This week, we've been examining air pollution,

:06:39.:06:41.

which affects many of us in the North West.

:06:42.:06:43.

Our biggest cities, Manchester and Liverpool,

:06:44.:06:47.

are both considering bans on the vehicles which

:06:48.:06:50.

it's believed cause most of our air pollution.

:06:51.:06:53.

In Europe, some places have been able to reduce vehicle emissions.

:06:54.:06:57.

And our Environment Correspondent Judy Hobson has been to see

:06:58.:07:00.

Zurich, Switzerland's largest city, and a similar size to Manchester.

:07:01.:07:07.

It welcomes shoppers, bankers and tourists, but not cars.

:07:08.:07:11.

Parking places are scarce and expensive.

:07:12.:07:15.

It's a deliberate policy to cut air pollution.

:07:16.:07:22.

leave your car at home, otherwise you are a bad citizen.

:07:23.:07:27.

No, if you want to take your car, you can.

:07:28.:07:29.

But we say we have another option that is really good.

:07:30.:07:34.

By that, he means cheap and efficient public transport.

:07:35.:07:38.

By law, every city centre resident has to live within 500 metres

:07:39.:07:43.

Half the urban population don't own a car.

:07:44.:07:54.

This square used to be full of parking places.

:07:55.:07:56.

Now cars are banned and its measures like this that have

:07:57.:07:58.

helped dissuade people from bringing their vehicles

:07:59.:08:00.

A two hour train ride away is Freigburg in Germany,

:08:01.:08:06.

The most popular way to commute is by bicycle.

:08:07.:08:13.

One area has been built and designed for people not to need cars.

:08:14.:08:17.

This is the district of Vauban, a sustainable area.

:08:18.:08:21.

You can have a car, but you have to pay 18,000 euros just

:08:22.:08:27.

All the shops are nearby, so day-to-day life is not very

:08:28.:08:32.

In summer, you see all the streets are in the

:08:33.:08:38.

street and they are playing together.

:08:39.:08:39.

I would have loved to live here as a kid.

:08:40.:08:42.

Like Zurich, it's an efficient public transport system which has

:08:43.:08:44.

helped make a difference, but also careful town planning.

:08:45.:08:49.

We don't have big shops at the border of the city,

:08:50.:08:52.

where you can buy milk and a television.

:08:53.:08:54.

We have shops in every residential area so you can

:08:55.:09:01.

Even here, more needs to be done to further reduce emissions,

:09:02.:09:10.

but Freigburg and Zurich show how urban air pollution can be reduced.

:09:11.:09:15.

It shows policies in places like Manchester

:09:16.:09:16.

much further, if we're to have any chance of cleaning up

:09:17.:09:21.

Football and Manchester City missed the chance to gain ground

:09:22.:09:30.

on Chelsea at the top of the Premier League tonight.

:09:31.:09:33.

They were held to a goalless draw by Stoke City at the Etihad Stadium.

:09:34.:09:37.

Kelechi Iheanacho went close in stoppage time.

:09:38.:09:41.

The point leaves City third, ten points off the pace

:09:42.:09:43.

Good evening. We have our first taste of spring today. Temperatures

:09:44.:09:55.

going into double figures and it was a beautiful end to the day. Our

:09:56.:10:00.

weather watcher photos, some lovely daffodils and flowers blooming.

:10:01.:10:04.

Through denied, most of us will have a dry night. There will be rain and

:10:05.:10:07.

showers moving across Cumbria, Lancashire into the early hours of

:10:08.:10:12.

Thursday morning. Breeze picking up a little bit. Temperatures no more

:10:13.:10:17.

than seven or 8 degrees. Cloud around, that cloud quickly shifting

:10:18.:10:24.

away and we are looking at some decent sunny spells going into

:10:25.:10:28.

lunchtime and certainly into the afternoon, as well. Temperatures in

:10:29.:10:31.

two double figures. Nine or ten, perhaps 11 or 12 Celsius. It feels

:10:32.:10:38.

much like spring. Going into Friday, we have got a weather front throwing

:10:39.:10:42.

in a bit of cloud and outbreaks of rain as we go throughout the day.

:10:43.:10:48.

Still quite mild, temperatures in double figures, certainly cloudier

:10:49.:10:51.

on Friday. With that, outbreaks of rain on and off throughout the day

:10:52.:10:56.

particularly around the coast and over the hills. Temperatures again

:10:57.:11:00.

getting up to about ten or 12 degrees. The weekend, mixed.

:11:01.:11:02.

outlook, staying mild and Nick has the bigger picture across the UK.

:11:03.:11:06.

Hello. Spring is in the air with temperatures reaching 14 or 15 in a

:11:07.:11:13.

few spots today as they will again over the next few days. Very

:11:14.:11:20.

pleasant in the sun. The daffodils were loving that in York. More

:11:21.:11:24.

places under blue sky tomorrow and dry. Tonight heavy showers moving

:11:25.:11:31.

across Scotland on strong to gale force winds, some may clip Northern

:11:32.:11:34.

Ireland. It's a mild night in southern England and South Wales but

:11:35.:11:39.

damp and drizzly, misty with coastal and hill fog elsewhere. Temperatures

:11:40.:11:48.

in between and dry: This damp weather hangs on from parts of the

:11:49.:11:54.

Channel Islands, to Cornwall. Elsewhere, it's getting brighter in

:11:55.:11:57.

South Wales and southern England. In England,

:11:58.:11:59.
