Robert Cox was stabbed to death by another resident

Today an inquest jury found that a lack of on-site support

and poor police interaction exacerbated the situation.

Sue Cox has spent years trying to get answers

He had mental health problems and ended up in this supported

:01:04.:01:08.

accommodation in Egerton Road, Bristol.

:01:09.:01:11.

On the night he died he'd gone swimming with his stepsister,

:01:12.:01:15.

niece and nephew, his dad and the two children

:01:16.:01:19.

and when he was left at 7:30 that evening,

:01:20.:01:22.

That evening was the 9th August 2013.

:01:23.:01:30.

Robert Cox was stabbed to death by another resident, Derek Hancock.

:01:31.:01:35.

Hancock had a history of paranoia, delusional behaviour

:01:36.:01:37.

and making false allegations of sexual harassment.

:01:38.:01:43.

On the night, Derek Hancock phoned police, making false allegations

:01:44.:01:49.

The police came and spoke to Hancock, but after they'd gone,

:01:50.:01:55.

His third and final call became the subject of an investigation

:01:56.:02:02.

Call handler Susan Akerman does say police "will deal with it".

:02:03.:02:08.

But Derek Hancock goes on: "I will take the law

:02:09.:02:11.

into my own hands because the law is not doing anything."

:02:12.:02:15.

"Well, that's entirely up to you Derek, OK?"

:02:16.:02:21.

She doesn't say don't do that, which I would think a call handler

:02:22.:02:26.

should be saying to anybody, mental health patient

:02:27.:02:28.

I understand the police watchdog agreed, but didn't think

:02:29.:02:33.

Ms Akerman's actions contributed to Robert Cox's death.

:02:34.:02:37.

But the watchdog did say she may have gone on to give them false

:02:38.:02:41.

and misleading information, which she denied.

:02:42.:02:46.

Ms Akerman didn't want to do an interview but has told

:02:47.:02:51.

the coroner here she'd had no training about mental health

:02:52.:02:54.

and she didn't know Hancock had such problems.

:02:55.:02:59.

Speaking about that call, she said he hadn't given

:03:00.:03:02.

the impression he was going to go on to do what he did,

:03:03.:03:06.

and that she took three or four calls a day from

:03:07.:03:09.

Police say they're committed to learning lessons.

But Sue Cox has had to fight for answers.

A serious case review was critical about the agencies involved.

But she forced an independent investigation which found the case

review wasn't sufficiently in-depth or inquisitive, and...

"The process floundered" and it's not surprising

It makes you question whether there are other cases

where the same questions, the same evidence, the same

problems are arising with serious case reviews.

The Bristol Adult Safeguarding Board has apologised

The group which ran Egerton Road said it's carried out risk

assessments and that Robert's death couldn't have been predicted.

But for Sue and her family, the battle isn't over.

We believe there is more work to be done to prevent future deaths.

Charlotte had attempted suicide before, after hearing

During her pregnancy she'd stopped taking the medication that

controlled her schizophrenia, believing it could harm her baby.

In fact, the review found there was "no definitive answer"

about the safe use of medication in this case.

What we will always do with patients is have an in-depth conversation

with them to say your mental health needs need to be the priority

because there is more risk to your child if your mental health

deteriorates than of the risk of medications transferred

Their bodies were found later after a search by police,

to stop anything similar ever happening again.

Since Charlotte Bevan's death in 2014, changes have already been

made to support pregnant women with severe psychiatric disorders.

Bristol, South Gloucestershire and North Somerset have managed

to secure ?1.2 million of government funding for a team of health

visitors and nurses to give women at high risk extra support

And funding has been agreed for another four-bed maternity

A girl reported missing from Wiltshire has been found.
