Now on BBC One, it's time for the news where you are. | :00:00. | :00:13.
A doctor has admitted that the life of a Kent mother-of-three might | :00:14. | :00:16.
well have been saved, if he had read her medical notes properly. | :00:17. | :00:19.
The admission sparked emotional scenes at the inquest | :00:20. | :00:20.
into the death of Sian Hollands, who was 25 when she died | :00:21. | :00:23.
at the Darent Valley Hospital in Dartford. | :00:24. | :00:25.
She suffered a pulmonary embolism just hours after doctors had decided | :00:26. | :00:28.
This afternoon, her family ran from the inquest | :00:29. | :00:36.
in Gravesend in tears, after hearing evidence | :00:37. | :00:37.
from Dr Kamran Khan, who had previously claimed he had not | :00:38. | :00:40.
had access to her notes. Peter Whittlesea has the details. | :00:41. | :00:42.
For Sian Hollands' family, the inquest inot her death | :00:43. | :00:45.
is all about uncovering the truth. Today, what was described in court | :00:46. | :00:48.
as significant new evidence highlighted why doctors failed | :00:49. | :00:49.
to diagnose the pulmonary embolism that killed her. | :00:50. | :00:51.
If they had just looked at the notes and treated her as a human being, | :00:52. | :00:55.
my sister would still be here. That is extremely hard. | :00:56. | :00:57.
When you heard that in court, you could not cope and had to leave. | :00:58. | :01:01.
I did, yeah. I did leave. | :01:02. | :01:02.
I did, yeah. I did leave. | :01:03. | :01:08.
Do you feel that you have finally heard the truth? | :01:09. | :01:13.
And I am glad I heard the truth, for my mum and for her children. | :01:14. | :01:21.
Throughout the inquiry, doctors claimed they had never | :01:22. | :01:23.
seen a nurse's notes, saying Sian was having | :01:24. | :01:25.
difficulty breathing and was experiencing chest pains, | :01:26. | :01:26.
which led to a wrong diagnosis. Today, those records were found and, | :01:27. | :01:29.
on the reverse side, were the handwritten notes by Dr | :01:30. | :01:33.
Kamran Khan, one of the medics who had treated Sian. | :01:34. | :01:40.
It led to a dramatic courtroom exchange... | :01:41. | :02:15.
In court, the hospital trust expressed their deepest | :02:16. | :02:17.
Her mother said these were empty words. | :02:18. | :02:22.
If they had turned that page or looked at that page, | :02:23. | :02:25.
things would have been so different. We would not be standing | :02:26. | :02:27.
here today, cos she would still be alive and well. | :02:28. | :02:32.
The inquest has now been adjourned until April 10, | :02:33. | :02:34.
when the coroner said he would record his verdict. | :02:35. | :02:42.
A BBC South East undercover investigation has revealed that | :02:43. | :02:44.
criminal gangs are using Facebook to sell fake cigarettes | :02:45. | :02:47.
which contain dangerously high levels of lead | :02:48. | :02:48.
and other toxic substances. Trading Standards says | :02:49. | :02:50.
it is a deeply-concerning trend in the South East, | :02:51. | :02:52.
which is endangering people's health and short-changing the economy. | :02:53. | :03:01.
A parliamentary aide to a Kent MP has appeared | :03:02. | :03:03.
at Westminster Magistrates Court, charged with two counts of rape. | :03:04. | :03:05.
Samuel Armstrong, who was chief of staff for the South Thanet MP | :03:06. | :03:08.
Craig Mackinlay, has been suspended since his arrest in October. | :03:09. | :03:11.
He has been bailed to appear before Southwark Crown Court next month. | :03:12. | :03:19.
The pilot and owner of a plane that ditched into the sea off the Sussex | :03:20. | :03:22.
coast yesterday says the engine cut out at an altitude | :03:23. | :03:25.
Ian Perry swam to shore with his passenger, after the light | :03:26. | :03:28.
aircraft came down into the water shortly after take-off | :03:29. | :03:31.
from Shoreham Airport, where he runs a flying school. | :03:32. | :03:33.
Insurance assessors have been examining the plane today. | :03:34. | :03:35.
It is likely to be written off, due to water damage. | :03:36. | :03:42.
Sentencing has begun for two violent criminals who poured boiling water | :03:43. | :03:45.
over a retired couple, in an horrific ordeal that lasted | :03:46. | :03:48.
for more than two hours. John and Janis Buswell | :03:49. | :03:49.
were tied up and tortured in their home in Halstead, | :03:50. | :03:52.
near Sevenoaks, last April. Both suffered life changing injuries | :03:53. | :03:54.
as a result of the attack, carried out by Daniel | :03:55. | :03:58.
Wallace and Kacey Adams. Mrs Buswell spent months | :03:59. | :04:02.
in intensive care. As Piers Hopkirk reports, | :04:03. | :04:03.
they say it has left them physically and mentally scarred, | :04:04. | :04:06.
feeling their lives have been destroyed. | :04:07. | :04:08.
Daniel Wallace and Kacey Adams. Violent criminals, with track | :04:09. | :04:10.
records in aggravated burglary. Their sustained two-hour | :04:11. | :04:12.
attack on retired couple, John and Janis Buswell, | :04:13. | :04:14.
so shocking it received nationwide media coverage. | :04:15. | :04:20.
This type of crime is incredibly rare. | :04:21. | :04:24.
It left seasoned detectives shocked by the level of violence used | :04:25. | :04:27.
in the committing of this offence. 26 April, 2016 is a day | :04:28. | :04:30.
that our victims live with daily. They were sat at their home address, | :04:31. | :04:33.
enjoying their evening together, when these persons forced their way | :04:34. | :04:35.
into the address and used what can only be described as a barbaric | :04:36. | :04:39.
level of violence against them. They were bound, beaten and had | :04:40. | :04:41.
boiling water poured over their heads and bodies, | :04:42. | :04:43.
while trying to steal money from these poor people. | :04:44. | :04:45.
It happened at the couple's home in Halstead. | :04:46. | :04:47.
It was a crime of such unprovoked violence, | :04:48. | :04:49.
it stunned people in this quiet village community. | :04:50. | :04:51.
I think we were all horrified when we heard about it, | :04:52. | :04:54.
because in a way, it could have happened to anyone. | :04:55. | :04:56.
Why it happened to them, I don't know, but you fear | :04:57. | :04:59.
for those people who live in fairly isolated places. | :05:00. | :05:10.
I do know the family. Yeah, it's pretty terrible what they | :05:11. | :05:13.
did, especially to his wife. I'd throw away the key. | :05:14. | :05:17.
So, they should get punished badly, yeah. | :05:18. | :05:22.
The court heard that Adams and Wallace had tortured the couple | :05:23. | :05:25.
After a prolonged period of threats and violence, they got them, | :05:26. | :05:29.
leaving their home with more than ?40,000 in cash. | :05:30. | :05:31.
Mrs Buswell's injuries were so severe, she later fell into a coma. | :05:32. | :05:34.
In a victim impact statement read to the court, she said: | :05:35. | :05:56.
The two guilty men will be sentenced here at 2pm on Monday. | :05:57. | :06:12.
They can expect very lengthy prison terms. | :06:13. | :06:24.
That is it from me. The national weather forecast | :06:25. | :06:26.
in a moment, but first, we can get the picture | :06:27. | :06:28.
in the South East, with Rachel Mackley. | :06:29. | :06:36.
Good evening. After a war into the week, it is a bit fresher. Some | :06:37. | :06:45.
scattered April showers on Saturday. The sunshine will be back on Sunday. | :06:46. | :06:54.
Temperatures in single figures overnight. Cool as we start the day | :06:55. | :07:06.
on Saturday. The chance of a shower during the day, particularly in the | :07:07. | :07:11.
morning. Some of these could be quite heavy. By the afternoon, | :07:12. | :07:17.
temperatures getting up to 12 Celsius along the coast, inland a | :07:18. | :07:25.
couple of degrees better. Saturday into Sunday, clear and dry. Some | :07:26. | :07:31.
mist and fog. In rural areas, temperatures dropping | :07:32. | :07:36.
Hello. There the weekend is here and Sunday looks like being the better | :07:37. | :07:43.
day of the two. We're going to start the weekend, a new month, with | :07:44. | :07:47.
sunshine and April showers. Those fade away overnight. Saturday night | :07:48. | :07:51.
could be chilly. It points to a fine day on Sunday. This picture was | :07:52. | :07:54.
taken by a weather watcher to end the day in Dundee. This one is | :07:55. | :07:57.
probably my | :07:58. | :07:58. |
