A doctor has admitted that the life of a Kent mother-of-three might

well have been saved, if he had read her medical notes properly.

The admission sparked emotional scenes at the inquest

into the death of Sian Hollands, who was 25 when she died

at the Darent Valley Hospital in Dartford.

She suffered a pulmonary embolism just hours after doctors had decided

This afternoon, her family ran from the inquest

in Gravesend in tears, after hearing evidence

from Dr Kamran Khan, who had previously claimed he had not

had access to her notes. Peter Whittlesea has the details.

For Sian Hollands' family, the inquest inot her death

is all about uncovering the truth. Today, what was described in court

as significant new evidence highlighted why doctors failed

to diagnose the pulmonary embolism that killed her.

If they had just looked at the notes and treated her as a human being,

my sister would still be here. That is extremely hard.

When you heard that in court, you could not cope and had to leave.

I did, yeah. I did leave.

I did, yeah. I did leave.

And I am glad I heard the truth, for my mum and for her children.

Throughout the inquiry, doctors claimed they had never

seen a nurse's notes, saying Sian was having

difficulty breathing and was experiencing chest pains,

which led to a wrong diagnosis. Today, those records were found and,

on the reverse side, were the handwritten notes by Dr

Kamran Khan, one of the medics who had treated Sian.

It led to a dramatic courtroom exchange...

In court, the hospital trust expressed their deepest

Her mother said these were empty words.

If they had turned that page or looked at that page,

things would have been so different. We would not be standing

here today, cos she would still be alive and well.

The inquest has now been adjourned until April 10,

when the coroner said he would record his verdict.

A BBC South East undercover investigation has revealed that

criminal gangs are using Facebook to sell fake cigarettes

which contain dangerously high levels of lead

and other toxic substances. Trading Standards says

it is a deeply-concerning trend in the South East,

which is endangering people's health and short-changing the economy.

A parliamentary aide to a Kent MP has appeared

at Westminster Magistrates Court, charged with two counts of rape.

Samuel Armstrong, who was chief of staff for the South Thanet MP

Craig Mackinlay, has been suspended since his arrest in October.

He has been bailed to appear before Southwark Crown Court next month.

The pilot and owner of a plane that ditched into the sea off the Sussex

coast yesterday says the engine cut out at an altitude

Ian Perry swam to shore with his passenger, after the light

aircraft came down into the water shortly after take-off

from Shoreham Airport, where he runs a flying school.

Insurance assessors have been examining the plane today.

It is likely to be written off, due to water damage.

Sentencing has begun for two violent criminals who poured boiling water

over a retired couple, in an horrific ordeal that lasted

for more than two hours. John and Janis Buswell

were tied up and tortured in their home in Halstead,

near Sevenoaks, last April. Both suffered life changing injuries

as a result of the attack, carried out by Daniel

Wallace and Kacey Adams. Mrs Buswell spent months

in intensive care. As Piers Hopkirk reports,

they say it has left them physically and mentally scarred,

feeling their lives have been destroyed.

attack on retired couple, John and Janis Buswell,

so shocking it received nationwide media coverage.

This type of crime is incredibly rare.

It left seasoned detectives shocked by the level of violence used

in the committing of this offence. 26 April, 2016 is a day

that our victims live with daily. They were sat at their home address,

enjoying their evening together, when these persons forced their way

into the address and used what can only be described as a barbaric

level of violence against them. They were bound, beaten and had

boiling water poured over their heads and bodies,

while trying to steal money from these poor people.

It happened at the couple's home in Halstead.

It was a crime of such unprovoked violence,

it stunned people in this quiet village community.

I think we were all horrified when we heard about it,

because in a way, it could have happened to anyone.

Why it happened to them, I don't know, but you fear

for those people who live in fairly isolated places.

I do know the family. Yeah, it's pretty terrible what they

did, especially to his wife. I'd throw away the key.

So, they should get punished badly, yeah.

The court heard that Adams and Wallace had tortured the couple

After a prolonged period of threats and violence, they got them,

leaving their home with more than ?40,000 in cash.

Mrs Buswell's injuries were so severe, she later fell into a coma.

In a victim impact statement read to the court, she said:

The two guilty men will be sentenced here at 2pm on Monday.

They can expect very lengthy prison terms.
