How a hospital missed a woman's deep vein thrombosis.

:00:00.:00:10.

Tributes to the two young men killed in a suspected hit-and-run.

:00:11.:00:23.

I'm more frost expected with freezing fog patches. Join me for

:00:24.:00:27.

the weather forecast. A pensioner who went to A

:00:28.:00:31.

with leg pains was sent home with painkillers

:00:32.:00:35.

and died hours later, Joan Hawes went to Addenbrooke's

:00:36.:00:37.

Hospital in July, correctly Mike Cartwright reports

:00:38.:00:40.

on how medics missed it. Joan Hawes, 77, who told nurses

:00:41.:00:48.

the pain in her knee But instead, ligament

:00:49.:00:51.

strain was misdiagnosed Arriving at the inquest,

:00:52.:00:58.

her daughters and a family friend. Steven, her son, drove his mother

:00:59.:01:05.

to Addenbrooke's that morning. She was in terrible pain,

:01:06.:01:08.

he said, her knee swollen. His mother repeatedly telling staff,

:01:09.:01:11.

a blood clot was to blame. The inquest told a nurse had

:01:12.:01:19.

ordered blood tests to look for a deep vein thrombosis -

:01:20.:01:22.

a clot, but when Joan saw nurse practitioner Andrew Carr,

:01:23.:01:25.

there was no information If he had known there

:01:26.:01:27.

were concerns about clots, he said he would have referred Joan

:01:28.:01:31.

to other medical staff. But instead, he gave her

:01:32.:01:34.

painkillers and sent her home. Later learning blood tests had been

:01:35.:01:42.

ordered, he cancelled them. Their solicitor today

:01:43.:01:44.

spoke for them. The family are understandably deeply

:01:45.:01:49.

distressed at what has happened to Mrs Hawes -

:01:50.:01:51.

an otherwise fit and healthy woman. They were looking to the inquest

:01:52.:01:56.

for answers and in some respects, the admissions from Addenbrooke's

:01:57.:01:59.

have gone a long way But the inquest still has

:02:00.:02:02.

an important function to perform and the conclusions will help

:02:03.:02:06.

further illuminate this tragedy. The hospital trust says

:02:07.:02:15.

an investigation was carried out to make sure lessons could be

:02:16.:02:17.

learned from what happened to Joan and a detailed action plan put

:02:18.:02:19.

in place to improve the way deep vein thrombosis is

:02:20.:02:23.

recognised and treated. Their mother died at home

:02:24.:02:25.

sometime during the night. Her death brought on by a blood clot

:02:26.:02:27.

that medical staff failed to find. She had been a healthy person. The

:02:28.:02:44.

coroner concluded she died of natural causes. He noted that when

:02:45.:02:49.

she arrived at the hospital she had not been referred to a member of the

:02:50.:02:55.

medical team. Nor heard the deep vein thrombosis beam depicted.

:02:56.:03:00.

So what exactly is Deep Vein Thrombosis

:03:01.:03:01.

Simply put, it's a blood clot in a deep vein, usually in the leg.

:03:02.:03:09.

Deep veins pass through the centre of the leg, through the muscles

:03:10.:03:11.

of the calf and thigh and carry the blood towards the heart.

:03:12.:03:16.

But when you have Deep Vein Thrombosis,

:03:17.:03:18.

the flow can be partially, or completely, blocked.

:03:19.:03:25.

Sometimes, there are no symptoms, but they can include - pain,

:03:26.:03:28.

swelling and tenderness in one or both sides of hip or legs,

:03:29.:03:31.

warm skin in the area of the clot, red skin at the back of the leg

:03:32.:03:35.

and below the knee and low-grade temperature.

:03:36.:03:38.

But I've spoken to an expert tonight, who says

:03:39.:03:40.

a doctor would always take into account two factors.

:03:41.:03:42.

They would also look at whether the person is on the pill.

:03:43.:03:46.

Have they been immobile for a long period?

:03:47.:03:48.

Have they had recent surgery on a hip or leg?

:03:49.:03:50.

Is there a family history of blood clots?

:03:51.:03:55.

If there are two factors, then the patient should be referred

:03:56.:03:57.

There is a risk if DVT is left untreated, that

:03:58.:04:06.

about one in ten people will develop a pulmonary embolism,

:04:07.:04:09.

which causes breathlessness and chest pain.

:04:10.:04:11.

I have spoken to a DVT patient this evening,

:04:12.:04:15.

who got the condition during a long-haul flight.

:04:16.:04:17.

She has spoken about how important it is to contact your GP right away.

:04:18.:04:20.

But also, give your GP the right information.

:04:21.:04:22.

In fact, she is so passionate about this message that she has

:04:23.:04:25.

launched an awareness campaign which can be found at

:04:26.:04:27.

Friends of the two men killed in an apparent hit and run incident

:04:28.:04:40.

near Peterborough have been paying tribute to them today.

:04:41.:04:42.

The victims, who have been named locally as Thomas Northam

:04:43.:04:44.

and Thomas Fletcher, were walking along the B1091 near

:04:45.:04:46.

Yaxley on Tuesday morning when they were struck by a BMW.

:04:47.:04:49.

Today, the local vicar said the men will be remembered

:04:50.:04:51.

in a service this Sunday. Stuart Ratcliffe reports.

:04:52.:04:58.

Two days on from this apparent hit and run

:04:59.:05:01.

and tributes are still being laid at the scene where two men named

:05:02.:05:03.

locally as Thomas Fletcher and Thomas Northam, were killed.

:05:04.:05:05.

It is no exaggeration to say that there is still a real sense

:05:06.:05:09.

of horror here in Yaxley at what took place on Tuesday.

:05:10.:05:13.

And from the people I have spoken to, it is clear that both young

:05:14.:05:17.

men were well known, well liked and well loved.

:05:18.:05:32.

Earlier this afternoon, I spoke to Thomas Fletcher's mum,

:05:33.:05:35.

who told me she still can't believe what has happened.

:05:36.:05:37.

And she told me that her son had only just returned from holiday

:05:38.:05:40.

The two men were walking between Yaxley and Facet

:05:41.:05:44.

The driver of which then fled the scene on foot.

:05:45.:05:48.

And for friends of the men, emotions are understandably

:05:49.:05:50.

Tom Northam was the best friend I've ever had.

:05:51.:05:53.

We spent birthdays together, went on holiday together,

:05:54.:05:55.

always talked, spent a lot of time together.

:05:56.:05:57.

Tom Northam was funny, he always knew how to make everyone smile.

:05:58.:06:00.

When I found out, I finished work and my friend told me straight away,

:06:01.:06:06.

It is not something I wanted to believe.

:06:07.:06:08.

And now it is something, I'm going to miss them both.

:06:09.:06:14.

Yaxley has a strong and vibrant community spirit.

:06:15.:06:15.

And this weekend, prayers will also be said in the village church.

:06:16.:06:21.

It is just desperate, desperate sadness.

:06:22.:06:23.

We didn't know the families at the time but now we know

:06:24.:06:27.

who they are, just a desperate sense of loss and just a desire to reach

:06:28.:06:30.

out to these families, we will be offering to support them

:06:31.:06:33.

and as I said already, we will be praying for

:06:34.:06:36.

them and doing whatever we can to support them.

:06:37.:06:40.

Earlier this week, a 38-year-old man was arrested, 80

:06:41.:06:42.

on suspicion of causing death by dangerous driving

:06:43.:06:45.

He is now being questioned by officers in Cambridgeshire.

:06:46.:06:56.

2017 holds some big changes to local government in Cambridgeshire.

:06:57.:06:58.

The devolution deal was signed late last year and, this May, the county

:06:59.:07:01.

It will have big impact on how local decisions are made,

:07:02.:07:05.

as our political reporter Tom Barton explains.

:07:06.:07:11.

We are all used to elections - for Parliament, for councils and,

:07:12.:07:14.

more recently, for police commissioners,

:07:15.:07:17.

but this May, in Cambridgeshirem, there will be an election that has

:07:18.:07:20.

At the moment, in most parts of the country, there are

:07:21.:07:27.

District councils look after issues like housing and waste collection,

:07:28.:07:35.

while bigger county councils are responsible for social

:07:36.:07:37.

But in Cambridgeshire, a new devolution deal will see

:07:38.:07:43.

the introduction of a third tier of local government.

:07:44.:07:46.

People say it is a new layer of government, but some

:07:47.:07:53.

faceless bureaucrat in Whitehall has been

:07:54.:07:54.

making these decisions. It will be a local voice now.

:07:55.:08:06.

That will be making decisions about some big issues.

:08:07.:08:11.

They will have the power to spend millions on building new homes.

:08:12.:08:14.

They will get control over the local training budget.

:08:15.:08:21.

And they will have power and money for new transport projects.

:08:22.:08:26.

What do the people who will elect the new mayor think?

:08:27.:08:29.

We have enough people in all the local councils, so why can't

:08:30.:08:32.

Especially if he knows Cambridge well, it could be good for us.

:08:33.:08:39.

What we are doing is adding a layer of cost,

:08:40.:08:42.

without providing a full democratic freedom.

:08:43.:08:47.

The mayor will work with a combined authority,

:08:48.:08:49.

made up of all the council leaders in Cambridgeshire and Peterborough.

:08:50.:08:54.

It met before Christmas, to start laying the groundwork

:08:55.:08:56.

Local business also has a voice and their

:08:57.:09:02.

representative sees the new mayor as a vitally-important figure.

:09:03.:09:08.

This is the spokesperson for Cambridgeshire and Peterborough.

:09:09.:09:10.

It gives us access to a global audience and, therefore, one person.

:09:11.:09:18.

We have seen how the Mayor of London is represented.

:09:19.:09:21.

All eyes are now focused on the election when Cambridgeshire

:09:22.:09:30.

will join the likes of Manchester and Sheffield

:09:31.:09:32.

as one of just half a dozen areas with a new devolved mayor.

:09:33.:09:42.

The government has announced a ?100 million investment

:09:43.:09:44.

in the Cambridge-based British Antarctic Survey.

:09:45.:09:47.

The money will be used to upgrade its Rothera Research Station,

:09:48.:09:49.

and allow the organisation to conduct more ambitious explorations.

:09:50.:09:52.

The base will be home to the new research ship,

:09:53.:09:55.

That is the late news from Look East.

:09:56.:10:00.

I will leave you with the weather, from Alex.

:10:01.:10:02.

It is another cold night around the region, with temperatures

:10:03.:10:05.

Also the added problem of freezing fog patches.

:10:06.:10:10.

They are likely to develop quite widely

:10:11.:10:11.

across the region overnight tonight, becoming quite dense in places.

:10:12.:10:14.

Also, some icy patches on untreated surfaces.

:10:15.:10:16.

So, minus 1-2 Celsius in many places, but it could be

:10:17.:10:18.

a degree or two lower in those frost-prone spots.

:10:19.:10:20.

So, we start the day tomorrow on a cold note.

:10:21.:10:23.

This weather system is coming in from the north-west.

:10:24.:10:32.

That will eventually bring some rain and some milder

:10:33.:10:34.

Freezing fog patches lingering through the morning in some places.

:10:35.:10:39.

Then, a little bit brighter, with perhaps some sunshine,

:10:40.:10:41.

before cloud increases from the west and this weather front brings

:10:42.:10:44.

But it will be a cold day, with high temperatures of just

:10:45.:10:51.

For the afternoon and evening, that rain spreading in from

:10:52.:10:54.

The national weather is coming up, but here is

:10:55.:10:57.

Milder, but cloudier, for the weekend.

:10:58.:11:02.

maybe nine Celsius. Here is Louise with the national picture.

:11:03.:11:12.

Won't it cold and frosty this morning? The coldest night of the

:11:13.:11:19.

winter so far in England. Down to minus 8

:11:20.:11:20.
